A rare case of positional vertebrobasilar ischemia with the retrograde flow of the vertebral artery
J Surg Case Rep. 2023 May 12;2023(5):rjad222. doi: 10.1093/jscr/rjad222. eCollection 2023 May.ABSTRACTPositional vertebrobasilar ischemia, also known as Bow hunter stroke, is typically caused by mechanical compression of the vertebral artery (VA). On the other hand, subclavian steal syndrome is incidentally detected by vertigo, syncope or loss of consciousness due to the steal phenomenon. A 61-year-old man suffered from near syncope when he turned his head to the left side. Although asymmetric arm blood pressure of the right dominant was observed, arm claudication was not detected. Computed tomographic angiography and magn...

Higher long-term cardiovascular morbidity after open surgery for intermittent claudication caused by infrainguinal atherosclerotic disease in patients with diabetes - a nationwide observational cohort study.
Conclusions: DM patients showed higher rates of MACE and AMI in propensity score adjusted analysis five years after planned infrainguinal open surgery for IC. Higher HbA1c was associated with MACE, stroke, and total mortality in patients with DM, whereas longer duration of DM was associated with major amputation. Left Subclavian Artery Revascularization for Thoracic Aortic Stentgrafting: Single Center Experience in 101 Patients
Introduction - Endovascular treatment of thoracic aortic pathology (TEVAR) requires a landing zone of at least 20mm in length, necessitating coverage of the left subclavian artery (LSA) in approximately 40% of the patients.1 Coverage of the LSA may increase the risk of ischaemic stroke, spinal cord ischaemia (SCI) and/or left arm claudication. Besides some endovascular options to preserve LSA flow, in the majority of cases the LSA is surgically revascularized by either a carotid-to-subclavian bypass graft or subclavian-carotid transposition.

A systematic review and meta-analysis of the effects of supervised exercise therapy on modifiable cardiovascular risk factors in intermittent claudication
The objective of this study was to determine the effects of supervised exercise therapy (SET) on modifiable cardiovascular risk factors in IC patients.

Supra-aortic vessel reconstruction in total  arch replacement for acute type A dissection: Comparison of en bloc and separate graft techniques.
CONCLUSIONS: The modified en bloc technique could achieve comparable early and mid-term outcomes to the branched graft technique in patients undergoing TAR for ATAAD. This approach may be an alternative technique for ATAAD patients with intact supra-aortic arch vessels. PMID: 30377019 [PubMed - as supplied by publisher]

Atherosclerosis knowledge - diagnosis and management in primary care.
CONCLUSIONS: Our study demonstrates that GPs' knowledge about atherosclerosis disease varies significantly depending on disease location. GPs diagnose correctly but need to be backed up for their management of patients with atherosclerosis. PMID: 30047837 [PubMed - as supplied by publisher]

Endovascular Versus Open Surgical Intervention in Patients with Takayasu's Arteritis: A Meta-analysis
Although medical treatment has advanced, surgical treatment is needed to control symptoms of Takayasu's arteritis (TA), such as angina, stroke, hypertension, or claudication. Endovascular or open surgical intervention is performed; however, there are few comparative studies on these methods. This meta-analysis and systematic review aimed to examine the outcome of surgical treatment of TA.

Endovascular Versus Open Surgical Intervention in Patients with Takayasu's Arteritis: A Meta-analysis.
CONCLUSION: This meta-analysis has shown a lower risk of restenosis with open surgical intervention than with endovascular intervention. Stroke was generally more common with open surgical intervention than with endovascular intervention. However, there were differences according to the location of the lesion, and the risk of stroke in open surgery is higher when the supra-aortic branches are involved rather than the renal arteries. Amputation Rates, Mortality, and Pre-operative Comorbidities in Patients Revascularised for Intermittent Claudication or Critical Limb Ischaemia: A Population Based Study.
CONCLUSION: The risk of amputation is particularly high during the first 6 months following revascularisation for CLI. IC patients have a benign course in terms of limb loss. Mortality in both IC and CLI patients is substantial. Revascularised CLI patients have different comorbidities from IC patients. PMID: 28797662 [PubMed - as supplied by publisher]

Efficacy and Safety of Transbrachial Access for Iliac Endovascular Interventions
Conclusion: Brachial artery access enables endovascular treatment of iliac artery disease in the majority of patients, although an adjunctive transfemoral access may be required. However, the high incidences of access site complications and cerebral events remain a significant limitation of the transbrachial approach.

Stroke from an External Carotid: Lesion Pattern and Mechanisms
Traditionally, patients with symptomatic external carotid stenosis present with neck or face pain, retinal ischaemic symptoms or jaw claudication. and rarely as ipsilateral cerebrovascular events. In this present case, our patient suffered a stroke from a paradoxical embolism from the external carotid, without involvement of the internal carotid artery. A plaque ulceration of the external carotid’s origin was the cause of this cerebral emboli. Duplex ultrasound showed a pathological left external carotid, with a floating thrombus in the internal carotid.
