Impact of pre ‐existing or new‐onset atrial fibrillation on 30‐day clinical outcomes following Transcatheter Aortic Valve Replacement: Results From The BRAVO 3 Randomized Trial
Conclusions: Prior or new‐onset AF within 30 days of transfemoral TAVR is noted in more than one‐third of patients. Despite greater baseline comorbidities than non‐AF patients, AF was not associated with greater risk of adjusted 30‐day outcomes. In the BRAVO 3 trial, early outcomes were similar regardless of anticoagulant strategy in each group. This article is protected by copyright. All rights reserved.

Duration of dual antiplatelet therapy following drug ‐eluting stent implantation: A systemic review and meta‐analysis of randomized controlled trials with longer follow up
ConclusionsIn the present analysis of RCTs with longer follow up (2 years or longer), S‐DAPT compared with L‐DAPT, was associated with higher rate of MI and lower rate of major bleeding without any significant difference in the rates of all cause mortality, cardiac mortality, ST, TVR, and stroke.

Does Surgical Ablation Energy Source Affect Long-Term Success of the Concomitant Cox Maze Procedure?
CONCLUSIONS: Concomitant CM procedures performed with cryothermal energy alone or combined with bipolar radiofrequency ablation are safe and exceedingly effective. Real world safety and efficacy of WATCHMAN LAA closure at one year in patients on dual antiplatelet therapy Results of the DAPT subgroup from the EWOLUTION all-comers study.
CONCLUSIONS: LAA closure with the WATCHMAN followed by DAPT therapy in a high risk patient population is safe. At one year the intervention is associated with a substantial risk reduction regarding ischemic stroke and major bleeding compared to the expected rate based on CHA2DS2-VASc and HAS-BLED scores. Long-term effectiveness and safety of transcatheter closure of patent foramen ovale compared with antithrombotic therapy: A meta-analysis of 6 randomised clinical trials and 3560 patients with reconstructed time-to-event data.
CONCLUSIONS: Compared with ATA, tPFOc reduces the risk of stroke at long-term follow-up but no benefit is observed in terms of TIA. Atrial fibrillation is higher after tPFOc, while major bleeding and migraine are comparable between groups. PMID: 29901447 [PubMed - as supplied by publisher]

A Systemic Review and Meta-Analysis of Sutureless Aortic Valve Replacement Versus Transcatheter Aortic Valve Implantation
ConclusionsOur meta-analysis of observational studies demonstrates that early mortality is lower after SU-AVR than after TAVI in selected patients. The rates of stroke and pacemaker implant are comparable between procedures; however, the incidence of paravalvular leak is higher after TAVI.

Comparing apples with oranges: The funfair of statistics
The CHA2DS2-VASc score estimates the risk of stroke in patients with atrial fibrillation.1,2 The CHA2DS2-VASc score accounts for congestive heart failure; hypertension; age of at least 75  years (doubled); diabetes mellitus; previous stroke, transient ischemic attack or thromboembolism (doubled); vascular disease; age 65 to 74 years; and sex category (female). Should the CHA2DS2-VASc score be considered as a categorical or a continuous factor, if the patients have an incremental in crease in risk of death, stroke, or bleeding?

Early left ventricular assist device-related strokes: Turn up the flow, turn down the embolism?
Despite technical and surgical advances in continuous-flow left ventricular assist devices (LVADs), stroke remains a major complication. In the eighth annual Interagency Registry for Mechanically Assisted Circulatory Support report,1 stroke dominates as the major cause of death between 6  months and 4 years of device support. Previously identified risk factors for stroke in LVAD patients include pump thrombosis, infection, blood pressure, gastrointestinal bleeding, and excessive antithrombotic treatment.

Optimizing Patient Discharge Management after Transfemoral Transcatheter Aortic Valve Implantation: The Multicentre European FAST-TAVI Trial.
CONCLUSIONS: We validated the appropriateness of a pre-specified set of risk criteria that allow for a safe and timely discharge. The rate of 30-day complications did not reveal any risk increase with this strategy compared with the reported outcomes in major TAVI trials and registries. Antiplatelet strategy after transcatheter aortic valve replacement: an updated meta-analysis.
CONCLUSIONS: The updated meta-analysis presents the evidence that post-TAVR DAPT increases bleeding events, with no benefit in survival and ischemic events, in comparison with SAPT. Nevertheless, it is currently difficult to evaluate by a meta-analysis the effectiveness of DAPT versus SAPT to prevent the valve thrombosis resulting in leaflet dysfunction , due to a limited number of existing publication. Probability of Uneventful Recovery after Elective Aortic Root Replacement for Aortic Aneurysm.
CONCLUSIONS: This study provides novel information regarding the probability of uneventful recovery while confirming low in-hospital mortality and stroke rates after elective aortic root replacement for aortic aneurysm. Mitral Valve Replacement Using Subvalvular Apparatus: A Systematic Review and Meta-Analysis.
CONCLUSION: MVR with the preservation of subvalvular apparatus improves clinical outcomes, such as long-term mortality, hospital length of stay, pneumonia, and bleeding. There is no significant difference in the risk of stroke, renal failure, or ICU length of stay. Non-Vitamin K Antagonist Oral Anticoagulant vs Warfarin for Post Cardiac Surgery Atrial Fibrillation.
CONCLUSIONS: NOACs are associated with a reduction in post-operative LOS, without excess bleeding or other short-term complications, compared to warfarin. These findings support the broader use of NOACs as a safe alternative to warfarin in patients with post-cardiac surgery AF at elevated stroke risk and acceptable bleeding risk. Impact of a surgical approach for implantation of durable left ventricular assist devices in patients on extracorporeal life support
ConclusionLIS for implantation of a durable LVAD in patients on va ‐ECLS implanted for cardiogenic shock is associated with less revision due to bleeding, less administration of blood products and absence of perioperative stroke, with no impact on survival.
