Severe thrombosis of bioprosthesis mitral valve after dabigatran.
Abstract A 41-year-old female was admitted to our hospital with an unidentified source of fever, dyspnea and dizziness. Transthoracic echocardiography demonstrated severe mitral valve regurgitation, and further examination with transesophageal echocardiography (TEE) revealed a 7 mm vegetation on the anterior mitral leaflet. Blood cultures were negative, and after 45 days of empiric 12 g/day ampicillin-sulbactam therapy, the vegetation was shown to have disappeared. Profile of infective endocarditis observed from 2003 - 2010 in a single center in Italy
Conclusion: S. aureus emerged as the leading causative organism of IE in a University hospital in northern Italy. Our study confirmed the high in-hospital mortality of IE, particularly if health-care associated, and the protective role of surgery.

Clinical significance of cerebrovascular complications in patients with acute infective endocarditis: a retrospective analysis of a 12-year single-center experience
Conclusions: IE can give rise to various forms of CVC, most frequently, acute ischemic brain lesions. In our study, major CVC was associated with high risk of mortality although total CVC was not significantly related to the risk of death in patients with IE.

Despite major advances in treating valvular heart disease, the in-hospital mortality (15–20%) and 1-year mortality (~=40%) for infective endocarditis (IE) has not improved even with modern antibiotics and surgical therapy. Conclusion: Aortic arch surgery after previous type A dissection repair can be performed with satisfactory early and mid-term results and low risk of stroke. Cerebral perfusion strategies likely contribute to positive outcomes. Favourable mid-term survival justifies performing such difficult reoperations.

The Association Between Surgical Indications, Operative Risk and Clinical Outcome in Infective Endocarditis: A Prospective Study From the International Collaboration on Endocarditis.
CONCLUSIONS: -Surgical decision-making in IE is largely consistent with established guidelines, although nearly one-quarter of patients with surgical indications do not undergo surgery. Operative risk assessment by STS-IE score provides prognostic information for survival beyond the operative period. S. aureus IE was significantly associated with non-surgical management. Intraparenchymal hemorrhage after heroin use
Heroin-associated stroke is a rare complication of use. Various proposed mechanisms of heroin-associated ischemic stroke have been proposed, including the following: cardioembolism in the setting of infective endocarditis, hypoxic ischemic brain injury in the setting of hypoxemia and hypotension, and infective arteritis or vasculitis from drug adulterants. A previously healthy 28-year-old woman presented to the emergency department with altered mental status and normal vitals after she was found wandering outside her apartment.

Utility of Brain MRI in Infective Endocarditis (S46.004)
CONCLUSIONS: In hospitalized patients with IE, ICH was associated with unfavorable clinical outcomes. However, in patients undergoing valve surgery, microbleeds on MRI were not associated with development of postoperative ICH. Preoperative brain MRI may result in a delay to surgery and did not have an impact on outcome.
