Low hemoglobin levels are associated with Bowman's capsule rupture and peritubular capillaritis in ANCA-associated renal vasculitis: a link of vascular injury to anemia?
CONCLUSION: Peritubular capillaritis and Bowman's capsule rupture correlate with low hemoglobin levels; this may indicate that histopathological lesions are linked with inflammatory vascular injury and relative erythropoietin deficiency in ANCA-associated renal vasculitis.PMID:37676636 | DOI:10.1007/s40620-023-01748-z

Dialysis after kidney transplant failure: how to deal with this daunting task?
J Nephrol. 2023 Sep 7. doi: 10.1007/s40620-023-01758-x. Online ahead of print.ABSTRACTThe best treatment for patients with end-stage kidney disease is kidney transplantation, which, if successful provides both a reduction in mortality and a better quality of life compared to dialysis. Although there has been significant improvement in short-term outcomes after kidney transplantation, long-term graft survival still remains insufficient. As a result, there has been an increase in the number of individuals who need dialysis again after kidney transplant failure, and increasingly contribute to kidney transplant waiting lists. ...

Renal-Tubular SGK1 is Required to Achieve Blood Pressure Surge and Circadian Rhythm
Am J Physiol Renal Physiol. 2023 Sep 7. doi: 10.1152/ajprenal.00211.2023. Online ahead of print.ABSTRACTBlood pressure (BP) follows a circadian pattern that rises during the active (morning surge) and decreases during the inactive (night dipping) phase of the day. Morning surge coincides with increased circulating Glucocorticoids and Aldosterone, ligands for GR and MR nuclear receptors respectively. Serum- and glucocorticoid-induced kinase (SGK1), a clock-controlled and GR- and MR-induced gene, plays a role in BP regulation in human and animal models. However, the role of SGK1 in BP circadian regulation has not yet been de...

Why is chronic kidney disease progressive? Evolutionary adaptations and maladaptations
Am J Physiol Renal Physiol. 2023 Sep 7. doi: 10.1152/ajprenal.00134.2023. Online ahead of print.ABSTRACTDespite significant advances in renal physiology, the global prevalence of chronic kidney disease (CKD) continues to increase. The emergence of multicellular organisms gave rise to increasing complexity of life resulting in tradeoffs reflecting ancestral adaptations to changing environments. Three evolutionary traits shape CKD over the lifespan: 1) variation in nephron number at birth, 2) progressive nephron loss with aging, and 3) adaptive kidney growth in response to decreased nephron number. Although providing plastic...

Intracellular signaling pathways of muscarinic acetylcholine receptor-mediated detrusor muscle contractions
In conclusion, cholinergic detrusor contraction and RhoA activation are mediated by both M2 and M3 receptors. Furthermore, whereas both Gαi and Gαq/11 proteins mediate UBSM contraction, the activation at the RhoA-ROCK pathway appears to be linked specifically to Gαi. These findings may aid the identification of more specific therapeutic targets for bladder dysfunctions.PMID:37675459 | DOI:10.1152/ajprenal.00261.2022

Elevated tacrolimus levels among COVID-19 kidney transplant recipients are associated with worse disease severity and clinical outcomes
CONCLUSION: Elevated tacrolimus levels occur in many renal transplant recipients with moderate to severe COVID-19 and are associated with worse clinical outcomes. Close drug monitoring is crucial to avoid toxicities and minimize over-immunosuppression complications.PMID:37675489 | DOI:10.5414/CN111137

Effect of canagliflozin in non-diabetic obese patients with albuminuria: A  randomized, double-blind, placebo-controlled trial
CONCLUSION: Canagliflozin 100 mg daily was well tolerated but did not significantly reduce UACR in non-diabetic obese patients with microalbuminuria. However, a significant temporary decline in eGFR might reflect a subtle reduction in glomerular hyperfiltration.PMID:37675488 | DOI:10.5414/CN111143

Chronic Interstitial Nephritis in Agricultural Communities: Observational and Mechanistic Evidence Supporting the Role of Nephrotoxic Agrochemicals
Clin J Am Soc Nephrol. 2023 Sep 6. doi: 10.2215/CJN.0000000000000312. Online ahead of print.ABSTRACTChronic interstitial nephritis in agricultural communities (CINAC) is an epidemic of kidney disease affecting specific tropical and subtropical regions worldwide and is characterized by progressive chronic kidney disease (CKD) in the absence of traditional risk factors such as hypertension and diabetes. CINAC prevalence is higher among young, male agricultural workers, but also affects women, children, and non-agricultural workers in affected areas. Biopsies from patients with CINAC across regions commonly demonstrate tubula...

Obinutuzumab in Frequently Relapsing and Steroid-Dependent Nephrotic Syndrome in Children
CONCLUSIONS: These results identified low-dose obinituzumab as a promising treatment option in children with steroid-dependent or frequently relapsing nephrotic syndrome, including those resistant to rituximab. The tolerance profile of obinutuzumab appears similar to that of rituximab, but hemogram and immunoglobulin levels should be monitored.PMID:37678236 | DOI:10.2215/CJN.0000000000000288

System-Level Strategies to Improve Home Dialysis: Policy Levers and Quality Initiatives
Clin J Am Soc Nephrol. 2023 Sep 6. doi: 10.2215/CJN.0000000000000299. Online ahead of print.ABSTRACTAdvocacy and policy change are powerful levers to improve quality of care and better support patients on home dialysis. While the kidney community increasingly recognizes the value of home dialysis as an option for patients who prioritize independence and flexibility, only a minority of patients dialyze at home in the United States. Complex system-level factors have restricted further growth in home dialysis modalities, including limited infrastructure, insufficient staff for patient education and training, patient-specific ...
