Defective Fibrillar Collagen Organization by Fibroblasts Contributes to Airway Remodeling in Asthma.
CONCLUSIONS: NLOM imaging enabled the structural assessment of the ECM, and the data suggest that airway remodeling in asthma involves the progressive accumulation of disorganized fibrillar collagen by airway fibroblasts. This study highlights the future potential clinical application of NLOM to assess airway remodeling in vivo. PMID: 30950644 [PubMed - as supplied by publisher]

International Study of the Epidemiology of Platelet Transfusions in Critically Ill Children With an Underlying Oncologic Diagnosis
Conclusions: Children with an underlying oncologic diagnosis receive nearly half of platelet transfusions prescribed by pediatric intensivists. Over half of these transfusions are prescribed at total platelet count greater than current recommendations. Studies must be done to clarify appropriate indications for platelet transfusions in this vulnerable population.

Association Between Acute Kidney Injury Duration and Outcomes in Critically Ill Children*
OBJECTIVES: Acute kidney injury occurs frequently in children during critical illness and is associated with increased morbidity, mortality, and health resource utilization. We aimed to examine the association between acute kidney injury duration and these outcomes. DESIGN: Retrospective cohort study. Settings: PICUs in Alberta, Canada. PATIENTS: All children admitted to PICUs in Alberta, Canada between January 1, 2015, and December 31, 2015. INTERVENTIONS: None. MEASUREMENTS AND MAIN RESULTS: In total, 1,017 children were included, and 308 (30.3%) developed acute kidney injury during...

Acute Liver Failure in Children
Pediatric acute liver failure is a rare diagnosis which can result in death or multiorgan system failure with a potential need for liver transplantation. The causative factors are many, but etiology, definitive pathophysiology, and directed therapies are still under investigation contributing to the difficulty in planning and providing both medical and nursing care. Clinical practice guidelines for acute liver failure are available for adult patients and through a national published position paper for pediatrics, but quality evidence is limited, especially with regard to testing and treatment recommendations, providing cha...

Energy Intake and Expenditure in Children With Heart Failure
Conclusions: Growth failure in heart failure is likely multifactorial and may be related to suboptimal energy intake secondary to exercise intolerance, malabsorption, and/or end-organ dysfunction due to impaired cardiac output. Doubly labeled water is a feasible method to assess total daily energy expenditure in children with heart failure.

Machine Learning to Predict Cardiac Death Within 1 Hour After Terminal Extubation*
Conclusions: Our long short-term memory model accurately predicted whether a child will die within 1 hour of terminal extubation and may improve counseling for families. Our model can identify potential candidates for donation after cardiac death while minimizing unnecessarily prepared operating rooms.

Cytokine Release Syndrome in the Pediatric Population and Implications for Intensive Care Management
Cytokine release syndrome represents a spectrum of disease varying from fever alone to multiorgan system failure. Most commonly seen following treatment with chimeric antigen receptor T  cell therapy, it is increasingly being described with other immunotherapies as well as following hematopoietic stem cell transplant. As its symptoms are nonspecific, awareness is key to timely diagnosis and initiation of treatment. Given the high risk of cardiopulmonary involvement, critical care providers must be familiar with the cause, symptoms, and therapeutic options. Current treatment modalities focus on immunosuppression and targeted cytokine therapy.

The European Society for Paediatric Nephrology study of pediatric renal care in Europe: comparative analysis 1998 –2017
ConclusionsThe 20-year follow-up analysis of pediatric renal care services in European countries revealed that pediatric nephrology has become a well-established subspecialty in pediatrics and nephrology in 2017. The ESPN will continue its efforts to further improve pediatric renal care for European children by harmonizing remaining disparities of renal care services.
