Bone Morphogenetic Protein 9 is a Mechanistic Biomarker of Portopulmonary Hypertension.
CONCLUSIONS: BMP9 is a sensitive and specific biomarker of PoPH, predicting transplant-free survival and the presence of PAH in liver disease. In rodent models, acquired deficiency of BMP9 signaling can predispose to or exacerbate PH, providing a possible mechanistic link between PoPH and heritable PAH. These findings describe a novel experimental model of severe PH that provides insight into the synergy between pulmonary vascular injury and diminished BMP9 signaling in the pathogenesis of PAH. Sovaldi - a "Revolution" in Clinical Care, or in Marketing and Public Relations?
DiscussionWashington Post/ Kaiser Health NewsOn May 12, 2014, in an article on the dilemma the drug's US price of $1000/ pill presents to Medicare, Richard Knox wrote this about a patient with the infection:Previous drug treatments didn't clear the virus from Bianco's system. But it's almost certain that potent new drugs for hep-C could cure him. In other words, the article asserted that Sovaldi and similar drugs cure nearly everyone with hepatitis C, even those not cured by previous treatment.  ReutersOn May 20, 2014, in an article about how US health insurers are balking at the price of Sovaldi, was this statement b...

Anaesthesia and intensive care for adult liver transplantation
Publication date: Available online 4 June 2015 Source:Anaesthesia & Intensive Care Medicine Author(s): Craig Beattie , Michael A. Gillies This review describes the preoperative assessment and listing of the patient for liver transplantation and some of the specific perioperative challenges this group of patients present to the anaesthetist. The principles of the early postoperative management in the intensive care unit are discussed as well as some of the signs of early graft dysfunction. Increasingly unwell patients receiving grafts that may come from more marginal donors present significant challenges to the tr...

Experience with Mechanical Circulatory Support for Medically Intractable Low Cardiac Output in a Pediatric Intensive Care Unit.
CONCLUSION: Mechanical circulatory support has played a critical role and has had a dramatic effect on survival in patients with medically intractable heart failure, particularly in recent years. Meticulous monitoring of acid-base status, laboratory findings, and early and liberal applications are recommended to improve outcomes without critical complication rates, particularly in neonates with single ventricle physiology. North American Society for Pediatric Gastroenterology, Hepatology, and Nutrition Position Paper on the Diagnosis and Management of Pediatric Acute Liver Failure
Pediatric acute liver failure (PALF) is a rare, rapidly progressive clinical syndrome with significant morbidity and mortality. The phenotype of PALF manifests as abrupt onset liver dysfunction, which can be brought via disparate etiology. Management is reliant upon intensive clinical care and support, often provided by the collaborative efforts of hepatologists, critical care specialists, and liver transplant surgeons. The construction of an age-based diagnostic approach, the identification of a potential underlying cause, and the prompt implementation of appropriate therapy can be lifesaving; however, the dynamic and rap...

Pediatric Acute Liver Failure
Pediatric acute liver failure is a rare process that results from many different diseases including toxin ingestion and drug overdose, infections, metabolic and genetic disorders, immune-mediated diseases, and ischemia. Up to 50% of children with acute liver failure will never have an underlying cause found. Early identification, supportive care, and disease-directed therapy are critical. For some children liver transplantation is needed for survival, but many children will recover with appropriate therapy, without the need for transplantation. Nonetheless, overall survival is approximately 50% without liver transplantatio...

Enhanced recovery after hepatectomy: a systematic review
Conclusion ERAS protocols in liver surgery appeared to be safe and effective. Recent recommendations from the ERAS group in liver surgery are the only ones published so far. Other studies evaluating ERAS components in liver surgery and recommendations from scientific societies are needed to spread this clinical care pathway.

The chronic ICU patient: Is intensive care worthwhile for patients with very prolonged ICU-stay ( ≥ 90 days)?
CONCLUSIONS: Only a small number of patients requires a very prolonged ICU stay. Two-third of patients survive the ICU stay and about one-third 1-Year. However, about 10% of patients have a remarkable recovery with a favourable overall outcome after 1-Year. PMID: 31494021 [PubMed - as supplied by publisher]
