Characteristics of Hospital Observation Services: A Society of Cardiovascular Patient Care Survey
Conclusions:Accredited chest pain centers have observation units whose LOSs and discharge rates are comparable to prior studies with utilization patterns that may serve as benchmarks for similar hospitals.

Our treatment strategy for critical limb ischemia.
Authors: Yamada T, Onishi K, Utsunomiya M, Nakamura M Abstract For the treatment of critical limb ischemia, collaboration with wound specialists and cardiologists performing revascularization is important. The foot care unit affiliated with related departments opened at our hospital in July 2010 for limb salvage, mainly under the leadership of the departments of cardiovascular internal medicineand plastic surgery. We have treated 194 patients up until October 2012. The primary diseases included 81 cases (87 limbs) of foot ulcer and gangrene, with complications of peripheral arterial diseases (PADs) in all cases. In...

Pediatric Resident Attitudes and Knowledge of Critical Congenital Heart Disease Screening
This study aimed to understand the knowledge, attitudes and confidence level related to critical congenital heart disease (CCHD) screening among pediatric residents. Pediatric residents were assessed via an anonymous survey related to CCHD guidelines and procedures as set out by the New York State Department of Health. The survey was emailed to pediatric residents at a large academic institution. A teaching intervention was performed after the initial survey, which was followed by an identical after-intervention survey. Forty-two residents responded to the pre-intervention survey (n = 42), and forty post-intervention (n...

Red Blood Cell Transfusion in the Postoperative Care of Pediatric Cardiac Surgery: Survey on Stated Practice
This study describes the stated red blood cell transfusion practice of physicians who treat postoperative pediatric cardiac surgery patients in intensive care units. A scenario-based survey was sent to physicians involved in postoperative intensive care of pediatric cardiac surgery patients in all Canadian centers that perform such surgery. Respondents reported their red blood cell transfusion practice in four postoperative scenarios: acyanotic or cyanotic cardiac lesion, in a neonate or an infant. In part A of each scenario, the patient was critically ill, but stabilized; in part B, the patient became unstable. Response r...

Regenerative medicine - From stem cell biology to clinical trials for pediatric heart failure
Progressive heart failure in children and young adults is a serious problem, with an overall mortality of 7% in the United States [1]. In the past, the vast majority of children with heart failure had either cardiomyopathy or myocarditis, however, with improved surgical survival of high risk neonates with congenital heart disease, the incidence of heart failure due to structural heart disease has increased dramatically. There are critical differences in the response to injury between the young and the mature heart, where in the former the potential for regeneration is greater, in comparison to the more limited response in ...

Potential and Limitations of Cochrane Reviews in Pediatric Cardiology: A Systematic Analysis
AbstractEvidence-based medicine has contributed substantially to the quality of medical care in pediatric and adult cardiology. However, our impression from the bedside is that a substantial number of Cochrane reviews generate inconclusive data that are of limited clinical benefit. We performed a systematic synopsis of Cochrane reviews published between 2001 and 2015 in the field of pediatric cardiology. Main outcome parameters were the number and percentage of conclusive, partly conclusive, and inconclusive reviews as well as their recommendations and their development over three a priori defined intervals. In total, 69 r...

Sepsis in a Panorama: What the Cardiovascular Physician Should Know.
Abstract Sepsis accounts for an estimated 30 million cases and 6 million deaths globally each year. According to a multidisciplinary task force convened by the Society of Critical Care Medicine and European Society of Intensive Care Medicine, sepsis is defined as life-threatening organ dysfunction due to a dysregulated host response to infection. Sepsis is a medical emergency, so much so that the World Health Organization made it a global health priority. Since patients with cardiovascular diseases have unique risk factors for sepsis, prompt and accurate diagnosis is critical. In this regard, the sepsis-specific S...

SCAI Shock Stages A Through E
The Society for Cardiovascular Angiography and Intervention (SCAI) has classified cardiogenic shock into stages from A through E. This was developed by a multidisciplinary team from cardiology (interventional, advanced heart failure and noninvasive), emergency medicine, critical care and cardiac nursing. Stage A: “At risk” for cardiogenic shock – large myocardial infarction or heart failure, but not yet in shock, with normal mentation and systolic blood pressure of 100 mm Hg or more. Stage B: “Beginning” shock – hypotension or tachycardia without features of hypoperfusion (normal mentation). Systo...

Pediatric Life Support: 2020 International Consensus on Cardiopulmonary Resuscitation and Emergency Cardiovascular Care Science With Treatment Recommendations.
e Support Collaborators Abstract This 2020 International Consensus on Cardiopulmonary Resuscitation and Emergency Cardiovascular Care Science With Treatment Recommendations (CoSTR) for pediatric life support is based on the most extensive evidence evaluation ever performed by the Pediatric Life Support Task Force. Three types of evidence evaluation were used in this review: systematic reviews, scoping reviews, and evidence updates. Per agreement with the evidence evaluation recommendations of the International Liaison Committee on Resuscitation, only systematic reviews could result in a new or revised treatment re...

Distance to Care, Rural Dwelling Status, and Patterns of Care Utilization in Adult Congenital Heart Disease
AbstractMany patients with adult congenital heart disease (ACHD) do not receive guideline-directed care. While distance to an ACHD center has been identified as a potential barrier to care, the impact of distance on care location is not well understood. The Oregon All Payer All Claims database was queried to identify subjects 18 –65 years who had a health encounter from 2010 to 2015 with an International Classification of Diseases-9 code consistent with ACHD. Residence area was classified using metropolitan statistical areas and driving distance was queried from Google Maps. Utilization rates and percentages were calcul...

Low Cardiac Output Syndrome After Cardiac Surgery: A Life-Threatening Condition from the Perspective of Pediatric Intensivists
Turk Kardiyol Dern Ars. 2022 Jun;50(4):284-292. doi: 10.5543/tkda.2022.21212.ABSTRACTLow cardiac output syndrome is a clinical picture insourcing from insufficient oxygen sup- ply to tissues so as to meet the metabolic demand, myocardial dysfunction, and cardiovas- cular insufficiency. Low cardiac output syndrome is seen in nearly 25% of pediatric patients who underwent corrective or palliative surgery due to congenital heart defects. It is a clinical condition occurring typically 6-18 hours after surgery in pediatric patients undergoing cardiac surgery and causes organ failure, prolonged hospital and intensive care hospit...
