I am both a patient with multiple chronic conditions and a patient advocate.  What some members of my care team(s) often forget is that I also have a life outside of the little bubble in which they see me.  I have had 3 kidney transplants, been on dialysis twice (both hemo and peritoneal), and was recently diagnosed with either Crohn’s or ulcerative colitis.  (The docs do not seem to know yet which one.  Given my eastern European background, I believe it is Crohn’s.)  While I have two other chronic conditions, I will not discuss them in this newsletter. Hemofiltration Is Not Associated With Increased Mortality in Children Receiving Extracorporeal Membrane Oxygenation*
Conclusions: In-hospital mortality was similar between children on extracorporeal membrane oxygenation with and without hemofiltration although hemofiltration appeared to be associated with a slight increase in the duration of extracorporeal membrane oxygenation and more liberal platelet transfusions.

Objective: To evaluate the course of urinary neutrophil gelatinase–associated lipocalin and urinary kidney injury molecule-1 levels in young children during extracorporeal membrane oxygenation and concomitant continuous hemofiltration. High-Volume Hemofiltration in Critically Ill Patients With Secondary Hemophagocytic Lymphohistiocytosis/Macrophage Activation Syndrome: A Prospective Study in the PICU
Objectives: Hemophagocytic lymphohistiocytosis, which includes primary (familial) and secondary hemophagocytic lymphohistiocytosis, is a fatal disease in children. Macrophage activation syndrome was defined in patients who met secondary hemophagocytic lymphohistiocytosis criteria with an underlying autoimmune disease. High-volume hemofiltration has shown beneficial effects in severe sepsis and multiple organ dysfunction syndrome. Use of Furosemide and Chlorothiazide Combination Continuous Infusion in Furosemide-Refractory Patients in the Pediatric Intensive Care Unit: A Retrospective Cohort Study
CONCLUSIONS: FCCCI resulted in a significant improvement in diuresis with achievement of negative fluid balance in pediatric ICU patients. FCCCI is a reasonable approach to aggressive diuresis in the pediatric patient, particularly in patients with limited access.
