New and Established Patient E/M Definitions (CMS vs. CPT®)
I get lot of requests from readers of The Happy Hospitalist asking how to know if a patient is a new or established patient.  Identifying the correct classification will prevent delays or denials of payment.  Many evaluation and management (E/M) codes are by definition described as new or established.  This lecture will attempt to explain various important clinical aspects related to this determination.  Keep in mind while the Centers For Medicare & Medicaid Services (CMS) uses  Current Procedural Terminology (CPT) codes, CMS definitions do not always agree with CPT® definitions.  This di...

The EEOC Loses (And Loses. And Loses.) In Federal Court Again
Walter Olson We keep reporting in this space about how federal courts have slapped down the long-shot lawsuits and activist legal positions advanced by the Obama administration’s Equal Employment Opportunity Commission (EEOC). In the Freeman case last year, a Maryland federal judge used such unflattering terms as “laughable,” “unreliable,” and “mind-boggling” to refer to Commission positions, while in the more recent Kaplan case, in which Cato filed a brief, a Sixth Circuit panel was only slightly more polite about the systematic shortcomings in the commission...

Perspectives Migration and health in southeast Asian history
The 19th century witnessed a rapid expansion in migration across Asia. In the century after 1840, around 20 million people from China and 30 million Indians moved to the growing cities and plantations of southeast Asia. In scale, their migration was comparable to movement across the Atlantic; in nature, migration within Asia was circular more often than it was permanent. Migrants set out in search of livelihood or to escape conditions at home. The least free among them served under contracts of indenture that could be little better than slavery; those more fortunate found opportunity in a world of rapid change.

Best Practices for Data Sharing in Phylogenetic Research
Introduction The amount of phylogenetic data has rapidly increased in its quality and availability over the past few decades. Additionally, phylogenetic matrices and trees are often based on and need to be linked to data on traits, geographic distribution, and genetic / genomic sequences. Despite the rapid growth in data generation, comparative data from published studies are too often unavailable, incomplete, or incompatible thereby greatly limiting reproducibility and expansion of existing studies 1,2. A greater focus on data integration and interoperability, even at the data collection phase of a project, allows for sca...

Carnegie Mellon developing online tools to detect and identify sex traffickers
(Carnegie Mellon University) A contract from the Defense Advanced Research Projects Agency is funding researchers at Carnegie Mellon University's School of Computer Science who are seeking to undercut sex traffickers by developing online tools that target a key vulnerability: the need to advertise. The three-year, $3.6 million CMU effort will develop machine learning algorithms for domain specific indexing and search, and will build on existing efforts by CMU researchers to analyze ads for sex services posted to websites.

I bow to my teacher: Fuzi, a topical herb?
Founder’s note: This is a guest post by NCNM student Yedidya Tabanpour, who writes quite glibly aboutvarious topics at his blog, and can also be found on Twitter.  Comments are appreciated on guest posts! Initial Exposure Each moment signifies an encounter or lack there of that spirals into pinnacles of movement. And, every movement a person makes is part of a larger healing transformation. Whether an encounter feels like smooth sailing or just back breaking labor, pay no heed. The real significance is the deeper eruption of meaning that concludes the passing of said event and that which carries on to the next pinna...

This Is What Happens After You Die
(Photo: © Lightning + Kinglyface and Jess Bonham) Most of us would rather not think about what happens to our bodies after death. But that breakdown gives birth to new life in unexpected ways, writes Moheb Costandi.“It might take a little bit of force to break this up,” says mortician Holly Williams, lifting John’s arm and gently bending it at the fingers, elbow and wrist. “Usually, the fresher a body is, the easier it is for me to work on.”Williams speaks softly and has a happy-go-lucky demeanor that belies the nature of her work. Raised and now employed at a family-run funeral home in north Texas, she has s...

Gathering Background for Effective Preclinical Study Design
By Mark Cunningham, PhD, Surpass, Inc. Preclinical in vivo research is valuable and often necessary in the evaluation and development of therapeutic products (drugs, medical devices, biologics, and combination products). It is very important that a preclinical study is properly designed, conducted, analyzed, interpreted, and reported to ensure the work provides scientifically valid information to support product evaluation and development, and that the research is conducted according to defined regulations (USDA, IACUC, FDA, etc.). Surpass has compiled a white paper outlining key steps to generating a well-designed precli...
