The Impact of Blunt Thoracic Aortic Injury on Patients Aged 80 Years or Older.
CONCLUSION: Patients aged 80 or older who have experienced BTAI tend to have worse outcomes than those without BTAI regardless of similar ISS. Therefore, because of the low incident of this injury in general and particularly in the elderly, only pooled data from multiple institutions will be able to shed light on the complex issues surrounding treatment decisions in a group of patients with an a priori limited life expectancy. A Quarter Century of Organ Protection in Open Thoracoabdominal Repair
Introduction: Thoracoabdominal aortic aneurysm (TAAA) remains a challenging problem. We sought to examine our experience with thoracic and thoracoabdominal aortic repairs over a 24-year period. Methods: Patient information was collected in a prospective database and analyzed retrospectively. Univariate and multivariable analysis was performed. Results: Between January 1991 and December 2014, we repaired 1896 descending thoracic (DTAA) or TAAA in 1795 patients. Mean age was 64.2 ± 13.8, and 702 (37%) were women. Thoracic Endovascular Aortic Repair with the Left Subclavian Artery Coverage: Why Not Save the Left Subclavian Artery?
We read with interest the article “Outcomes of thoracic endovascular aortic repair and subclavian revascularization techniques” by Zamor and colleagues1 in the March 2015 issue of the Journal of the American College of Surgeons. The authors reported that thoracic endovascular aortic repair (TEVAR) with left subclavian artery (LSA) coverage is associated with an increased risk of stroke and upper extremity ischemia, and LSA revascularization is associated with lower rates of perioperative adverse events.

Propensity-matched analysis of minimally invasive mitral valve repair using a nationwide surgical database
Conclusions MICS-mitral via RT was successful without compromising the clinical outcomes. Although the operation time and postoperative bleeding should be improved, an RT approach is safe in appropriately selected patients, especially those <60 years of age or treated in a high-volume center.

Surgical repair of an extracranial carotid aneurysm
We present a case of a patient with an aneurysm of the carotid bifurcation who was treated conventionally with reconstruction of the carotid bifurcation using a venous patch. Endovascular Management of Rupture in Acute Type B Aortic Dissections.
CONCLUSION: With 16% in hospital mortality and 8% early major complications, this study confirms the feasibility of TEVAR for r-ABAD with a lower peri-operative morbidity and mortality rate compared with open surgery. Open retrograde endovascular stenting for left common carotid artery dissection secondary to surgical repair of acute aortic dissection: a case report and review of the literature
A 30-year-old male presented with an acute aortic artery dissection (Stanford Type A) and underwent total arch replacement using a stented elephant trunk technique. One month later, the patient developed dissections in the innominate and left common carotid artery (CCA). The innominate artery dissection caused occlusion in the right internal carotid artery (ICA) and a major stroke. Dissection of the left CCA progressed and extended to the bifurcation site. Antegrade access for a left carotid intervention was deemed as difficult because of the previously implanted stent and the additional risks of embolic events and dissection enlargement.

Transapical Endovascular Aortic Aneurysm Repair in a Patient with Shaggy Aorta Syndrome
We report a case of a 68-year-old man with a large saccular aneurysm (70 mm) of the aortic arch. Although abundant atherosclerotic plaques or mural thrombi are generally considered to be a contraindication for endovascular repair, the patient’s multiple comorbidities and anatomical limitations with a patent internal thoracic artery graft adjacent to the aneurysm made him unfit for open repair. Transapical deployment of the endograft through the less diseased ascending aorta, with a concomitant chimney graft and carotid-carotid bypass, was performed, without evident stroke or embolism.
