Outcomes of Cardiac Pacing in Adult Patients After a Fontan Operation
Conclusions More than one-third of adult Fontan patients referred to Mayo Clinic had pacemaker implantation. Epicardial leads were associated with high rate of pacemaker reinterventions but similar DRC rates in comparison to endocardial leads.

Effect of Milrinone Infusion on Pulmonary Vasculature and Stroke Work Indices: A Single-Center Retrospective Analysis in 69 Patients Awaiting Cardiac Transplantation
ConclusionMilrinone improved left ventricular stroke work indices to a greater extent than right ventricular stroke work indices and had beneficial effects on right ventricular net input impedance, predominantly via augmentation of left ventricular stroke volume and passive unloading of the pulmonary circuit. Patients who had the worst biventricular performance benefited the most from chronic milrinone infusion.

Dynamic Changes in Aortic Vascular Stiffness in Patients Bridged to Transplant  With Continuous-Flow Left Ventricular Assist Devices
Conclusions Aortic stiffness is markedly increased immediately post-OHT among patients bridged with CF-LVADs, with attenuation of this increased stiffness over the first year after transplant. These results suggest that aortic vascular properties are dynamic and may be influenced by alterations in flow pulsatility. As more patients are supported with CF-LVADs and as newer pump technology attempts to modulate pulsatility, further research examining the role of alterations in flow patterns on vascular function and the potential resultant systemic sequelae are needed. Thirty years of heart transplantation at the University Medical Centre Utrecht.
CONCLUSION: Over the past 30 years, patient and donor demographics and underlying diseases have shifted substantially. Furthermore, the increase in waiting time due to lack of available donor hearts has led to a rise in the use of LVADs as bridge to transplant. Importantly, an improvement in survival rates is found over time which could be explained by better immunosuppressive therapy and improvements in follow-up care. Relationship between aetiology and left ventricular systolic dysfunction in hypertrophic cardiomyopathy
Conclusions In adults with HCM, LV systolic dysfunction is more frequent in those with rare phenocopies. When combined with age at presentation, it is a marker for specific aetiologies and is associated with poorer long-term survival.

Outcomes in Adult Fontan Patients with Atrial Tachyarrhythmias
Conclusions CA and FC operations are associated with less recurrence of atrial tachyarrhythmia compared with AAD. Atrial tachyarrhythmias are more likely to recur in patients with a longer history of the arrhythmia, and are associated with more adverse events. Early referral to a specialty center for these interventions should be considered.

Abstract Sickle cell disease is a risk factor for cerebrovascular accidents in the pediatric population. This risk is compounded by hypoxemia. Vitamin K antagonists for stroke prevention in hemodialysis patients with atrial fibrillation: a systematic review and meta-analysis
Conclusion Our meta-analysis revealed a trend for a reduction of the risk of ischemic stroke in hemodialysis patients with AF treated with VKA. The true protective effect may have been underestimated, owing to inclusion of low-risk patients not expected to benefit from anticoagulation and to suboptimal anticoagulation. However, assessment of the overall effect of VKA in hemodialysis patients should also take into account the increased risk of bleeding, in particular of hemorrhagic stroke. Whether new oral anticoagulants provide a better benefit–risk ratio in hemodialysis patients should be the subject of future trials.

Donor Cause of Death in Heart Transplantation and Its Effect on Post-Transplant Survival —A UNOS Database Review
Introduction: There are no large scale analyses in the United States that assess the effect of donor cause of death (CoD) on survival in heart transplantation (HTX). We sought to evaluate whether the various CoDs cause significant short- and long-term survival differences in HTX recipients. Methods: We evaluated the UNOS registry for all adult HTX recipients from 2001 to 2014 and their adult donors. Recipients with RVAD, BiVAD, or TAH were excluded from our analysis.
