We report the first case of eosinophilic pleural effusion due to Anisakis spp. infection in a 39-years-old European subject hospitalized for worsening dyspnoea and abdominal and thoracic pain. Lung CT scan showed bilateral pleural effusion; thoracentesis revealed significant eosinophilia (45%), with normal eosinophils in the blood. Microbiological tests on pleural effusion were negative for bacteria, SARS-CoV-2, tuberculosis, fungi and parasites. Case report: JAKi and TNFi dual therapy is a potential treatment strategy for difficult-to-treat rheumatoid arthritis
This report describes a successful case of a 48-year-old woman with difficult-to-treat RA who treated with Tofacitinib combined with adalimumab. She had been on glucocorticosteroid for a long time, but had persistent joint pain and fatigue. At more than one year of follow-up, her Disease Activity Score for 28-joint counts based on the erythrocyte sedimentation rate (DAS28-ESR) remained in complete remission, and she discontinued her glucocorticosteroid medications. Primary Tuberculosis Presenting as a Soft Tissue Mass in the Sacroiliac Joint of an Immunocompetent Patient: A Case Report
A 46-year-old immunocompetent Somali female immigrant presented with a 2-year history of progressive left hip pain refractory to conservative management. Magnetic resonance imaging revealed a large mass arising from her left sacroiliac joint and extending into her left gluteus maximus. Biopsy and culture identified an abscess of unknown etiology, prompting an incision and drainage. This abscess was later identified as primary tuberculosis, for which she was placed on an antitubercular regimen and had complete resolution of her symptoms. Peritoneal tuberculosis mimicking peritoneal carcinomatosis in a patient with gallbladder carcinoma

Physical examination detected pain in the right hypochondrium. Abdominal ultrasonography showed a gallbladder mass with multiple gallstones. Magnetic resonance cholangiopancreatography demonstrated irregular thickening of the gallbladder wall, possibly infiltrating the liver parenchyma (Figure  1). Computed tomography (CT) of the abdomen, carried out in the context of the workup for metastasis, showed a thickening of peritoneal surfaces that was suggestive of peritoneal carcinomatosis.
