Does arterial cannulation site for aortic dissection repair impact surgical outcomes?
CONCLUSIONS: This study found no statistically significant different in rates of stroke or other complications based on cannulation site. Femoral arterial cannulation thus remains a safe and efficient choice for arterial cannulation in the repair of acute type A aortic dissection.PMID:37065559 | PMC:PMC10089872 | DOI:10.21037/jtd-22-576

National Trends, Risk Factors, and Outcomes of Acute In-hospital Stroke After Lung Transplantation in the United States: Analysis of the United Network for Organ Sharing Registry
Chest. 2023 Apr 11:S0012-3692(23)00503-2. doi: 10.1016/j.chest.2023.04.007. Online ahead of print.ABSTRACTBACKGROUND: Lung transplantation is the definitive treatment for end-stage lung failure. However, there have been no large, long-term studies on the impact of acute in-hospital stroke in this population.RESEARCH QUESTION: What are the trends, risk factors, and outcomes of acute stroke in patients undergoing lung transplantation in the United States?STUDY DESIGN AND METHODS: We identified adult first-time, isolated lung transplant (LTx) recipients from the United Network for Organ Sharing (UNOS) database, which comprehe...

Ineffective triggering in patients with acute brain injury undergoing invasive mechanical ventilation
Conclusion: Ineffective triggering was found very common in patients with acute brain injury, especially those receiving craniotomy for brain tumor. Low respiratory drive was independently associated with severe ineffective triggering at expiratory phase.PMID:36997326 | DOI:10.4187/respcare.10596

Reply: Association between triple therapy and major adverse cardiovascular events in COPD patients
We thank P. Almagro and P. Martinez-Camblor for their interest in our work and for their salient comments. While we agree that there is no universally accepted definition of major adverse cardiovascular event (MACE), the US Food and Drug Administration (FDA) and the European Medicines Agency include nonfatal myocardial infarction and stroke as well as cardiovascular (CV) mortality in their MACE definition [1]. Importantly, this definition includes both haemorrhagic and ischaemic stroke [1–4]. In our study, we followed this classical "three-point" definition of MACE (including haemorrhagic strokes). While this definit...

Prognostic Implications of Obstructive Sleep Apnea in Acute Coronary Syndrome by Obesity Status
Chest. 2023 Feb 8:S0012-3692(23)00173-3. doi: 10.1016/j.chest.2023.02.001. Online ahead of print.ABSTRACTBACKGROUND: A close relationship exists between obstructive sleep apnea (OSA) and obesity. The impact of obesity on the prognostic significance of OSA in patients with acute coronary syndrome (ACS) remains unclear.RESEARCH QUESTION: Whether the effects of OSA on subsequent cardiovascular events in ACS patients vary with obesity status?STUDY DESIGN AND METHODS: This is a prospective cohort study. Patients aged 18 to 85 years and hospitalized for ACS were consecutively enrolled and underwent portable sleep monitoring afte...

The safety of immediate extubation, and factors associated with delayed extubation, in cardiac surgical patients receiving fast-track cardiac anesthesia: An integrative review
CONCLUSION: Evidence pertaining to treatment outcomes associated with FTCA and IE is weak. Observational studies cannot determine causation. Large multicentre randomized control trials are required to determine the safety of IE. Although numerous factors have been associated with delayed extubation, several studies do not describe how or which factors were selected for examination. Therefore, certain factors may have yet to be evaluated. Future studies should comprehensively define all factors under investigation.PMID:36741304 | PMC:PMC9854388 | DOI:10.29390/cjrt-2022-037

Association of obstructive sleep apnoea with cardiovascular events in women and men with acute coronary syndrome
Conclusions In hospitalised ACS patients, OSA was associated with increased risk of subsequent events, particularly among women. Female patients with ACS should not be neglected for OSA screening and dedicated intervention studies focusing on women with ACS and comorbid OSA should be prioritised.
