Risks of Cardiovascular Adverse Events and Death in Patients With Previous Stroke Undergoing Emergency Noncardiac, Nonintracranial Surgery: The Importance of Operative Timing
(Abstracted from Anesthesiology 127(1):9–19, 2017) The purpose of this study was to assess the risk of adverse events after emergency noncardiac, noncranial surgery in association with time elapsed after ischemic stroke by retrospectively analyzing the data from a nationwide study population from Denmark that covered patients older than 20 years undergoing the aforementioned emergency surgeries between 2005 and 2011. The primary outcomes measured were the 30-day all-cause mortality and a combined end point of 30-day major adverse cardiovascular events (MACEs), such as nonfatal myocardial infarction and ischemic stroke, ...

Development of a Reactive Oxygen Species-Sensitive Nitric Oxide Synthase Inhibitor for the Treatment of Ischemic Stroke.
In this study, the nitrone 5was designed such that its decomposition product is a NOS inhibitor, 6, effectively leading to NOS inhibition specifically at the site of ROS production. The ability of 5to spin-trap radicals and decompose to 6 was observed using EPR and LC-MS/MS. The pro-drug concept was tested in vitro by measuring cell viability and 6 formation in SH-SY5Y cells subjected to oxygen glucose deprivation (OGD). 5 was found to be more efficacious and more potent than PBN, and was able to increase phospho-Akt while reducing nitrotyrosine and cleaved caspase-3 levels. 6 treatment, but not 5, was found to decrease NO...

Endovascular Treatment of Acute Ischemic Stroke Under General Anesthesia: Predictors of Good Outcome
Background: The choice of anesthetic technique, general anesthesia (GA) versus Monitored Anesthesia Care, may impact the outcome of patients undergoing endovascular treatment of acute ischemic stroke (AIS). The aim of this study was to identify the factors associated with good discharge outcome in patients receiving GA for AIS. Materials and Methods: Electronic medical records of patients above 18 years old who underwent endovascular treatment of AIS under GA at a Comprehensive Stroke Center from 2010 to 2014 were reviewed. Good outcome was defined as discharge modified Rankin Score 0 to 2 and poor outcome as modified...

Anesthetic management of endovascular treatment for acute ischemic stroke: Influences on outcome and complications.
CONCLUSIONS: General anesthesia and conscious sedation are both safe alternatives for anesthetic management of patients submitted to endovascular thrombectomy. No anesthetic management is universally recommended and hopefully the ongoing randomized clinical trials will shed some light on the best approach; meanwhile, the choice of anesthesia should be based on the patient's individual characteristics. Regarding neuroprotection, hemodynamic stability is currently the most important strategy, as no pharmacological method has been proven effective in humans. Stroke Thrombectomy: Anaesthesiologic Aspects.
Abstract Ischemic Stroke is a focal neurological deficit based on an acute ischemia. rt-PA lysis and thrombectomy are therapeutic options. Thrombectomy can be performed under local anaesthesia, sedation or general anaesthesia. Early studies clearly favoured sedation compared to general anaesthesia regarding neurological outcome. The following prospective, randomized trials could not detect harmful effects of general anaesthesia. Nonetheless in anaesthesia care of ischemic stroke patients many (patho-)physiological and pharmacological aspects should be taken into account for the best neurological outcome. PMID...

The effect of positive end-expiratory pressure on stroke volume variation: an experimental study in dogs.
Abstract Stroke volume variation (SVV) may be affected by ventilation settings. However, it is unclear whether positive end-expiratory pressure (PEEP) affects SVV independently of the effect of driving pressure. We aimed to investigate the effect of driving pressure and PEEP on SVV under various preload conditions using beagle dogs as the animal model. We prepared three preload model, baseline, mild and moderate hemorrhage model. Mild and moderate hemorrhage models were created in 9 anesthetized, mechanically ventilated dogs by sequentially removing 10 mL/kg, and then an additional 10 mL/kg of blood, respectively....

General anesthesia versus monitored anesthesia care during endovascular therapy for vertebrobasilar stroke
CONCLUSION: For patients who underwent endovascular therapy for vertebrobasilar occlusion strok caused by occlusions in the posterior circulation, MAC appears to be as effective as GA. However, MAC is associated with bigger final infarct volume. Future studies are warranted to confirm our findings.PMID:33841679 | PMC:PMC8014376
