Amnioinfusion for chorioamnionitis.
CONCLUSIONS: There is insufficient evidence to fully evaluate the effectiveness of using transcervical amnioinfusion for chorioamnionitis and to assess the safety of this intervention or women's satisfaction. We did not identify any trials that used transabdominal amnioinfusion. The evidence in this review can neither support nor refute the use of transcervical amnioinfusion outside of clinical trials. We included one small study that reported on a limited number of outcomes of interest in this review. The numbers included in this review are too small for meaningful assessment of substantive outcomes, where reported. For t...

Exploring Opioid-Sparing Multimodal Analgesia Options in Trauma: A Nursing Perspective
In conclusion, opioid-sparing multimodal analgesia addresses concerns associated with high doses of opioids, and many pharmacologic and nonpharmacologic options are available to implement this strategy. Nurses play key roles in comprehensive patient assessment; administration of patient-focused, opioid-sparing, multimodal analgesia in trauma; and monitoring for safety concerns.

Effect of preoperative oral analgesics on pulpal anesthesia in patients with irreversible pulpitis —a systematic review and meta-analysis
ConclusionsThe administration of preemptive analgesics can induce superior intraoperative analgesia for patients with irreversible pulpitis. However, strategies such as co-administration of certain types of analgesics and anesthetic solution might be predictors of treatment effect. Additionally, there was no association between different timing and dosage of analgesics and treatment effect.Clinical relevanceWhen compared to placebo, preemptive oral analgesics are superior in achieving anesthetic success in inflamed pulp.

Clinical update on benefit versus risks of oral paracetamol alone or with codeine: still a good option?
CONCLUSION: Due to its safety and tolerability profile paracetamol remained a first-line treatment in many international guidelines. Alone and with codeine it is a safe and effective option in adults, whilst NSAIDs are obviously less safe as alternatives, given the risk of potentially fatal GI and CV adverse effects. A longitudinal assessment of miR-122 and GLDH as biomarkers of drug-induced liver injury in the rat.
CONCLUSIONS: Together, this study demonstrates that both GLDH and miR-122 could be used during preclinical drug-development as complementary biomarkers to ALT to increase the chance of early detection of hepatotoxicity. PMID: 27978773 [PubMed - as supplied by publisher]

Paracetamol (acetaminophen) with or without codeine or dihydrocodeine for neuropathic pain in adults.
CONCLUSIONS: There is insufficient evidence to support or refute the suggestion that paracetamol alone, or in combination with codeine or dihydrocodeine, works in any neuropathic pain condition. PMID: 28027389 [PubMed - as supplied by publisher]

Perceptions vs. evidence: therapeutic substitutes for antipsychotics in patients with dementia in long-term care.
CONCLUSION: Perceptions of LTC clinicians regarding substitutes for antipsychotics in LTC patients with dementia vary widely and are often discordant with published evidence. PMID: 28080146 [PubMed - as supplied by publisher]

Aspirin (single dose) for perineal pain in the early postpartum period.
CONCLUSIONS: We found low-quality evidence to suggest that single dose aspirin compared with placebo can increase pain relief in women with perineal pain post-episiotomy. Very low-quality evidence also suggested that aspirin can reduce the need for additional analgesia, without increasing maternal adverse effects. Evidence was downgraded based on study limitations (risk of bias), imprecision, and publication bias or both. RCTs excluded breastfeeding women so there is no evidence to assess the effects of aspirin on neonatal adverse effects or breastfeeding. With international guidance recommending mothers initiate breastfeed...

Regular use of acetaminophen or acetaminophen-codeine combinations and prescription of rescue therapy with non-steroidal anti-inflammatory drugs: a population-based study in primary care.
Conclusion These findings show that regular use of acetaminophen or acetaminophen-codeine combinations may reduce the need for NSAIDs to treat OA-related pain. PMID: 28318320 [PubMed - as supplied by publisher]

A Review of Adjunctive CNS Medications Used for the Treatment of Post-Surgical Pain
AbstractInadequate post-operative pain management can have significant impacts on patients ’ quality of life. Effective management of acute pain after surgery is important for early mobilization and discharge from hospital, patient satisfaction, and overall well-being. Utilizing multimodal analgesic strategies has become the mainstay of acute post-operative pain management. A comprehens ive search was performed, assessing the published or otherwise publically available literature on different central nervous system (CNS) drugs [excluding opioids, non-steroidal anti-inflammatory drugs (NSAIDs), and acetaminophen] and thei...

Paracetamol (acetaminophen) for chronic non-cancer pain in children and adolescents.
CONCLUSIONS: There was no evidence from randomised controlled trials to support or refute the use of paracetamol (acetaminophen) to treat chronic non-cancer pain in children and adolescents. We are unable to comment about efficacy or harm from the use of paracetamol to treat chronic non-cancer pain in children and adolescents. We know from adult randomised controlled trials that paracetamol, can be effective, in certain doses, and in certain pain conditions (not always chronic). This means that no conclusions could be made about efficacy or harm in the use of paracetamol (acetaminophen) to treat chronic non-cancer pain in ch...

Intravenous paracetamol dosing guidelines for pain management in (pre)term neonates using the paediatric study decision tree.
CONCLUSIONS: A loading dose 20 mg/kg, followed by 10 mg/kg/6h is recommended for 32-44 weeks' neonates, which is supported by short-term safety. For neonates < 32 weeks, a loading dose of 12 mg/kg and a maintenance dose of 6mg/kg/6h seems to lead to the target Cssmean, though additional clinical studies are needed to support its safety. Orthodontic treatment-induced temporal alteration of jaw-opening reflex excitability
The impairment of orofacial motor function during orthodontic treatment needs to be addressed, because most orthodontic patients experience pain and motor excitability would be affected by pain. In the present study, the temporal alteration of the jaw-opening reflex excitability was investigated to determine if orthodontic treatment affects orofacial motor function. The excitability of jaw-opening reflex evoked by electrical stimulation on the gingiva and recorded bilaterally in the anterior digastric muscles was evaluated at 1 (D1), 3 (D3), and 7 days (D7) after orthodontic force application to the teeth of right side; mo...

A review on environmental monitoring of water organic pollutants identified by EU guidelines
Publication date: 15 February 2018 Source:Journal of Hazardous Materials, Volume 344 Author(s): João C.G. Sousa, Ana R. Ribeiro, Marta O. Barbosa, M. Fernando R. Pereira, Adrián M.T. Silva The contamination of fresh water is a global concern. The huge impact of natural and anthropogenic organic substances that are constantly released into the environment, demands a better knowledge of the chemical status of Earth’s surface water. Water quality monitoring studies have been performed targeting different substances and/or classes of substances, in different regions of the world, using different types of sampling strategi...
