Anti-vascular endothelial growth factor combined with intravitreal steroids for diabetic macular oedema.
CONCLUSIONS: Combination of intravitreal anti-VEGF plus intravitreal steroids does not appear to offer additional visual benefit compared with monotherapy for DMO; at present the evidence for this is of low-certainty. There was an increased rate of cataract development and raised intraocular pressure in eyes treated with anti-VEGF plus steroid versus anti-VEGF alone. Patients were exposed to potential side effects of both these agents without reported additional benefit. The majority of the evidence comes from studies of bevacizumab and triamcinolone used as primary therapy for DMO. There is limited evidence from studies u...

Intramuscular versus oral corticosteroids to reduce relapses following discharge from the emergency department for acute asthma.
CONCLUSIONS: There is insufficient evidence to identify whether IM corticosteroids are more effective in reducing relapse compared to oral corticosteroids among children or adults discharged from an ED or equivalent acute care setting for acute asthma. While we found no statistical differences, patients receiving IM corticosteroids reported fewer adverse events. Additional studies comparing the effectiveness of IM versus oral corticosteroids could provide further evidence clarity. Furthermore, there is a need for studies comparing different IM corticosteroids (e.g. IM dexamethasone versus IM methylprednisone) and different...

Promising Clinical Efficacy and Toxicity Profile of Isatuximab Based Regimens for Treatment of Newly Diagnosed and Relapsed/Refractory Multiple Myeloma: A Systematic Review
Conclusion:In RRMM pts, ISA as a single agent has shown weaker efficacy when compared to combination regimens i.e. ORR 21% vs. 58%. The best result was seen when ISA was used in combination with CFZ demonstrating an ORR of 80%. In NDMM pts, combination regimens have shown excellent efficacy with an ORR of 87%. Nausea and fatigue were the major AEs reported with the monotherapy while neutropenia, leucopenia, and lymphopenia were the major AEs reported with the combination regimens. Further studies involving a larger population are required to gather evidence in favor of the improved efficacy and to evaluate AEs.DisclosuresN...

Efficacy and Tolerability of Daratumumab in Heavily Pretreated AL Amyloidosis: A Systematic Review
ConclusionDaratumumab monotherapy is associated with deep and prompt hematological responses in patients with heavily pretreated AL amyloidosis, at the standard dosing regimens used for multiple myeloma, with a favorable safety profile. Furthermore, daratumumab performed well in patients with cardiac amyloidosis even though there is an increased risk of volume overload and infusion related morbidity. Given the high incidence of peripheral neuropathy with bortezomib, cardiotoxicity with carfilzomib based regimens in amyloidosis patients, daratumumab appears to be a suitable alternative. It has already been approved for rela...

Efficacy of Lenalidomide Based Three Drug Versus Two Drug Regimen for Relapsed Multiple Myeloma: A Systematic Review and Meta-Analysis of Randomized Trials
Conclusion:Daratumumab and carfilzomib in Len based three drug regimens have better clinical efficacy when compared to lenalidomide and dexamethasone based two drug regimens among relapsed and refractory multiple myeloma patients. There was no difference between the ORR of ixazomib based combination regimen and LenD but higher number of patients achieved deeper responses.DisclosuresNo relevant conflicts of interest to declare.

Efficacy of Bortezomib Based Regimens and Disease Monitoring in Light Chain Multiple Myeloma: A Review of Literature
ConclusionThere is a paucity of data on LCMM in the literature. V has shown superior efficacy in LCMM patients over non-V regimens with ORR > 95%. Moreover, V showed better PFS at 2 years when used with bendamustin compared to VD (95% vs. 25%). Similarly, V with B showed better OS i.e. 90% at 2 years. Moreover, sFLC levels were more sensitive in indicating the disease and predicting PFS and OS compared to uFLC levels, hence monitoring of LCMM patients should include serum assays.DisclosuresNo relevant conflicts of interest to declare.

Efficacy and Safety Profile of Novel Alkylators in Combination Regimens for Patients with Multiple Myeloma: A Review of Literature
ConclusionOur study demonstrated that combination regimen (BVD) has shown superior efficacy with an ORR of >90% when compared to other combination regimens. Furthermore, BVD was well tolerated in patients with grade 3 and 4 toxicities < 20%. Moreover, the combination of B with M when used as conditioning regimen for ASCT showed superior efficacy than M alone (90.6% vs. 81.2%). However, there is a paucity of data regarding this and future randomized prospective trials are needed.DisclosuresNo relevant conflicts of interest to declare.

Impact of Newer Agents on Progression Free Survival of Multiple Myeloma in the 2nd 3rd and 4th Line Setting- a Systematic Review
Conclusion: Regimen consisting of KRD has the highest median PFS in MM patients with multiple prior lines of therapy followed by IRD based regimen. Limitations of our analysis include a heterogeneous patient population and exclusion of data with daratumumab based regimen because of unavailability of median PFS specifically in the 2nd 3rd and 4th line setting in the published clinical trials.DisclosuresNo relevant conflicts of interest to declare.

Evidence-based dexamethasone dosing in malignant brain tumors: what do we really know?
ConclusionsEvidence on the safety and efficacy of different DXM doses in malignant brain tumor patients is scarce and conflicting. Best available evidence suggests that low DXM doses may be noninferior to higher doses in certain circumstances, but more comparative research in this area is direly needed, especially in light of the increasing importance of immunotherapy for brain tumors.

Glucocorticoid for Hearing Preservation After Cochlear Implantation: A Systemic Review and Meta-analysis of Animal Studies
Conclusions: Administering either systemic or topical glucocorticosteroids had a significant effect on preserving low and high-frequency hearing. Topical administration was equally effective across a range of concentration levels and provided maximal hearing preservation when applied 120 minutes before implantation. The effect of systemic treatment was achieved with high doses, equivalent to 26 mg of dexamethasone per day in humans. No significant effect was found with the use of drug-eluting electrodes and more studies are needed to characterise the utility and efficacy of this administration method.
