2023 Canadian Thoracic Society Guideline on Pharmacotherapy in Patients With Stable COPD
Chest. 2023 Sep 6:S0012-3692(23)05275-3. doi: 10.1016/j.chest.2023.08.014. Online ahead of print.ABSTRACTChronic obstructive pulmonary disease patient care must include confirming a diagnosis with postbronchodilator spirometry. Because of the clinical heterogeneity and the reality that airflow obstruction assessed by spirometry only partially reflects disease severity, a thorough clinical evaluation of the patient should include assessment of symptom burden and risk of exacerbations that permits the implementation of evidence-informed pharmacologic and nonpharmacologic interventions. This guideline provides recommendations...

Inflammation and immunomodulatory therapies influence the relationship between ATP-binding cassette A1 membrane transporter-mediated cholesterol efflux capacity and coronary atherosclerosis in rheumatoid arthritis
CONCLUSION: ABCA1-CEC associated with decreased atherosclerosis in patients with below-median baseline and time-averaged CRP and bDMARD use. Conversely, ABCA1-CEC associated with increased plaque in those with higher CRP, corticosteroid users, methotrexate nonusers, and bDMARD nonusers. While in well-treated and controlled disease ABCA1-CEC appears atheroprotective, in uncontrolled RA its action may be masked or fail to counteract the inflammation-driven proatherogenic state.PMID:37520890 | PMC:PMC10371792 | DOI:10.1016/j.jtauto.2023.100209

Management of patients with brain metastases from NSCLC without a genetic driver alteration: upfront radiotherapy or immunotherapy?
Ther Adv Med Oncol. 2023 May 29;15:17588359231175438. doi: 10.1177/17588359231175438. eCollection 2023.ABSTRACTLung cancer is the second most common cancer and the most common cause of cancer-related death in the United States. Brain metastases (BM) are detected in 21% of patients with lung cancer at the time of diagnosis and are the sole metastatic site in 35% of patients with stage IV disease. The best upfront therapy for non-small-cell lung cancer depends on both tumor programmed death 1 ligand-1 (PD-L1) expression and the presence or absence of a targetable genetic alteration in genes such as epidermal growth factor re...

Specific immune response to mRNA vaccines against COVID-19 in patients receiving allogeneic stem cell transplantation for myeloid malignancy was altered by immunosuppressive therapy
CONCLUSIONS: The results showed that both humoral and cellular adaptive immune responses of HSCT recipients to the SARS-CoV-2 vaccine were significantly suppressed by corticosteroid therapy. Specific response to the vaccine was significantly affected by the length of the interval between HSCT and vaccination. Vaccination as early as 5 months after HSCT can lead to a good response. Immune response to the vaccine is not related to age, gender, HLA match between HSC donor and recipient, or type of myeloid malignancy. Vaccine efficacy was dependent on well-reconstituted CD4+ T cells, at six months after HSCT.PMID:37216792 | DO...

Lowering the Dose of Corticosteroid Regimen in Kidney Transplantation: Is It Effective in Decreasing Post-operative Surgical Complications?
CONCLUSION: Post-operative corticosteroid dosage decrement through our protocol would lessen the serious associated postoperative surgical complications, without negative impacts on overall patient and graft survival.PMID:37089059 | DOI:10.22037/uj.v20i.7493

Sweet syndrome and erythema nodosum in a young patient with ulcerative colitis
We describe a case of a 26-year-old with a past medical history of ulcerative colitis on chronic prednisone presenting to the hospital with a 3-week ulcerative colitis flare with concomitant erythema nodosum and SS. While SS is well described, this inflammatory skin condition is uncommon in the setting of chronic anti-inflammatory medication.PMID:37091768 | PMC:PMC10120566 | DOI:10.1080/08998280.2023.2167050
