2023 Canadian Thoracic Society Guideline on Pharmacotherapy in Patients With Stable COPD
Chest. 2023 Sep 6:S0012-3692(23)05275-3. doi: 10.1016/j.chest.2023.08.014. Online ahead of print.ABSTRACTChronic obstructive pulmonary disease patient care must include confirming a diagnosis with postbronchodilator spirometry. Because of the clinical heterogeneity and the reality that airflow obstruction assessed by spirometry only partially reflects disease severity, a thorough clinical evaluation of the patient should include assessment of symptom burden and risk of exacerbations that permits the implementation of evidence-informed pharmacologic and nonpharmacologic interventions. This guideline provides recommendations...

Use of thiols and implications for the use of inhaled corticosteroids in the presence of oxidative stress in COPD
CONCLUSIONS: Further clinical trials of antioxidant agents with and without ICS are needed to better understand the place of thiol-based drugs in the treatment of patients with COPD.PMID:37517999 | DOI:10.1186/s12931-023-02500-8

Systemic interventions for treatment of Stevens-Johnson syndrome (SJS), toxic epidermal necrolysis (TEN), and SJS/TEN overlap syndrome
CONCLUSIONS: When compared to corticosteroids, etanercept may result in mortality reduction. For the following comparisons, the certainty of the evidence for disease-specific mortality is very low: corticosteroids versus no corticosteroids, IVIG versus no IVIG and cyclosporin versus IVIG. There is a need for more multicentric studies, focused on the most important clinical comparisons, to provide reliable answers about the best treatments for SJS/TEN.PMID:35274741 | PMC:PMC8915395 | DOI:10.1002/14651858.CD013130.pub2

Anticoagulant-related nephropathy: Focus on novel agents. A review
Adv Clin Exp Med. 2022 Feb 25. doi: 10.17219/acem/142657. Online ahead of print.ABSTRACTAnticoagulant-related nephropathy (ARN) is a novel and not well-studied cause of acute kidney injury (AKI). The prevalence of ARN varies significantly between studies and is estimated at 20% in patients treated with warfarin. Patients with ARN have a significantly higher mortality risk and an increased risk of chronic kidney disease (CKD). Unexplained AKI with hematuria are clinical manifestations of ARN. In most cases, ARN is diagnosed within the first 2 months of anticoagulant therapy, but later ARN occurrence is possible. Among the s...

The triumvirate of NF- κB, inflammation and cytokine storm in COVID-19
Int Immunopharmacol. 2021 Oct 15;101(Pt B):108255. doi: 10.1016/j.intimp.2021.108255. Online ahead of print.ABSTRACTThe coronavirus disease (COVID-19) has once again reminded us of the significance of host immune response and consequential havocs of the immune dysregulation. The severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) inflicts severe complications to the infected host, including cough, dyspnoea, fever, septic shock, acute respiratory distress syndrome (ARDs), and multiple organ failure. These manifestations are the consequence of the dysregulated immune system, which gives rise to excessive and unatten...

Pharmacological agents for adults with acute respiratory distress syndrome.
CONCLUSIONS: We found insufficient evidence to determine with certainty whether corticosteroids, surfactants, N-acetylcysteine, statins, or beta-agonists were effective at reducing mortality in people with ARDS, or duration of mechanical ventilation, or increasing ventilator-free days. Three studies awaiting classification may alter the conclusions of this review. As the potential long-term consequences of ARDS are important to survivors, future research should incorporate a longer follow-up to measure the impacts on quality of life. Diagnosis and Treatment of Alcoholic Hepatitis: A Systematic Review
Alcoholic hepatitis (AH) occurs in about one‐third of individuals reporting long‐term heavy alcohol use. It is associated with high short‐term mortality, economic burden, and hospital resources utilization. We performed this systematic review to (i) describe clinical characteristics and genomics associated with the risk of AH; (ii) discuss role and limitations of liver biopsy and prognostic scoring systems; (iii) summarize evidence regarding the currently available therapies including liver transplantation; and (iv) outline emerging therapies with areas of unmet need. Literature search was performed for studies publi...
