Interventional prevention of paradoxical embolism as the gold standard: End of discussion?
Biomed Pap Med Fac Univ Palacky Olomouc Czech Repub. 2021 Jun 14. doi: 10.5507/bp.2021.036. Online ahead of print.ABSTRACTParadoxical embolism is one of the predominant causes of cryptogenic stroke and interventional secondary prevention, i.e., closure of the patent foramen ovale (PFO), is a much discussed issue. This review aims to provide a complex perspective on this topic, aggregates and comments on the available data and current guidelines. Several large trials were performed, some of which proved the superiority of PFO closure over pharmacotherapy while others have not. Studies detecting significant superiority of in...

Toxicology and the biological role of methanol and ethanol: Current view.
CONCLUSIONS: The impact of ethanol and methanol on the body is shown as case reports, along with a discussion on the possible implication of alcohol in Alzheimer's disease and antidotal therapy for methanol poisoning. The role of ethanol in cancer and degenerative disorders seems to be underestimated given the current knowledge. Treatment in case of poisoning is another issue that remains unresolved even though effective protocols and drugs exist. PMID: 26006090 [PubMed - as supplied by publisher]

Off-pump versus on-pump coronary artery bypass grafting surgery in high-risk patients: PRAGUE-6 trial at 30 days and 1 year.
CONCLUSION: Off-pump surgical revascularization in patients with high operative risks can significantly reduce the incidence of major postoperative complications during the first 30 days. There was no statistically significant difference in the incidence of these complications after 1 year. PMID: 26740049 [PubMed - as supplied by publisher]

The 2017 hormone therapy position statement of The North American Menopause Society
Abstract: The 2017 Hormone Therapy Position Statement of The North American Menopause Society (NAMS) updates the 2012 Hormone Therapy Position Statement of The North American Menopause Society and identifies future research needs. An Advisory Panel of clinicians and researchers expert in the field of women's health and menopause was recruited by NAMS to review the 2012 Position Statement, evaluate new literature, assess the evidence, and reach consensus on recommendations, using the level of evidence to identify the strength of recommendations and the quality of the evidence. The Panel's recommendations were reviewed and a...

The Impact of Ionospheric and Geomagnetic Changes on Mortality from Diseases of the Circulatory System
We investigate the impact of solar activity changes on mortality from cardiovascular causes of death in the period 1994-2011 in the Czech Republic. This period coincides with the time of solar cycle no. 23 and the surrounding minima when there was an unusually low level of solar activity. We use long-period daily time series of numbers of deaths by cause, solar activity indices (the relative sunspot number, and the intensity of solar radio flux), geomagnetic indices (Kp —the planetary index that indicates the fluctuation rate of horizontal components of the geomagnetic field, the Auroral Electrojet, and the disturbance s...

Autonomic control of blood circulation in patients undergoing elective carotid endarterectomy.
CONCLUSION: In the early postoperative period, healing processes are occurring and the sympatho-vagal interaction is probably still unbalanced. Given the considerable clinical potential of BRS and HRV measurement, further short-term and, more importantly, long-term investigations are needed. PMID: 29086771 [PubMed - as supplied by publisher]

Vulnerable atherosclerotic plaque - review of current concepts and advanced imaging.
Authors: Spacek M, Zemanek D, Hutyra M, Sluka M, Taborsky M Abstract Atherosclerosis is the most common cause of both carotid and coronary steno-occlusive disease. Rupture of an atherosclerotic plaque may lead to the formation of an overlying thrombosis resulting in complete arterial occlusion or downstream embolism. Clinically, this may manifest as a stroke or acute myocardial infarction, the overall leading causes of mortality and disability in developed countries. In this article, we summarize current concepts of the development of vulnerable plaque and provide an overview of commonly used imaging methods that m...

Safety of early discharge in low risk patients after acute ST-segment elevation myocardial infarction, treated with primary percutaneous coronary intervention. Open label, randomized trial.
CONCLUSION: This study confirms that early discharge within 48-72 h in selected low risk patients after STEMI treated with successful primary PCI is feasible and safe, with outcomes comparable to the later discharge. This strategy applies to more than a quarter of all STEMI patients. PMID: 30181666 [PubMed - as supplied by publisher]

Midterm outcomes of mini-invasive surgical and hybrid ablation of atrial fibrillation.
CONCLUSIONS: . Combination of mini-invasive surgical and endocardial treatment (two-stage hybrid procedure) is a safe and effective method for the treatment of isolated (lone) AF. This procedure provided good midterm outcomes. PMID: 30214077 [PubMed - as supplied by publisher]

Oxidative Modified Fibrinogen in Cardiovascular Diseases
ConclusionsThe results show that the degree of impairment of fibrinogen functions in the cardiovascular diseases is related to the level of oxidative stress. Characterization of oxidative fibrinogen modification and its precise meaning to the function of fibrinogen in hemostasis appears to be extremely helpful to better understanding of thrombotic/bleeding complications linked with various cardiovascular diseases.AcknowledgementsThis work was supported by the Ministry of Health, Czech Republic, no. 00023736, by the Academy of Sciences, Czech Republic no. Association of 18bp insertion/deletion polymorphism, at −2549 position of VEGF gene, with diabetic vascular complications in type 2 diabetes mellitus
ConclusionsThe multivariate logistic regression analysis showed that the D allele in the promoter region of the VEGF gene is an independent risk factor of DR irrespective of other laboratory and clinical variables in T2DM patients. Our study suggests that I/D allele in the studied gene is associated with HF and strokes.
