A case of undifferentiated carcinoma of the bladder treated by combination therapy of surgery, local radiation, and immune checkpoint inhibitors
We present the case of an 81-year-old woman complaining of bladder irritation. Computed tomography (CT) and magnetic resonance imaging (MRI) revealed a bulky bladder tumor, which was diagnosed as cT4aN0M0. A transurethral resection of the bladder tumor revealed pT2 or higher urothelial carcinoma. The patient underwent total cystectomy with an ileal conduit diversion. Histological examination revealed a primary undifferentiated bladder carcinoma with a sarcoma component, pT3aN0M0. Postoperative adjuvant chemotherapy with gemcitabine plus carboplatin was administered. However, this regimen was discontinued after one course d...

Enhancing Mass Spectrometry-Based MHC-I Peptide Identification Through a Targeted Database Search Approach.
Abstract MHC-bound peptide ligands dictate the activation and specificity of CD8+ T- cells-based and thus are important for devising T-cell immunotherapies. In recent times, advances in mass spectrometry (MS) have enabled the precise identification of these peptides, wherein MS/MS spectra are compared against a reference proteome. Unfortunately, matching immunopeptide MS/MS to reference proteome databases is hindered by inflated search spaces attributed to the number of matches that need to be considered due to a lack of enzyme restriction. These large search spaces limit the efficiency with which MHC-I peptides a...

Integrating Database Search and De Novo Sequencing for Immunopeptidomics with DIA Approach.
Abstract Identification of tumor-specific antigens (neoantigens) is needed for development of effective cancer immunotherapy and a good source for such antigens are the pools of HLA-bound peptides presented exclusively by the tumor cells. Mass spectrometry (MS) has evolved as the method of choice for the exploration of the human immunopeptidome (HLA class-I and class-II peptides). The key challenge is to deal with the low abundance of these peptides. Data-independent acquisition (DIA) technology promises to capture the low abundance data. However, the high number of fragments ions generated from multiple peptide p...

Anaphylaxis:: After the Emergency Department
Emerg Med Clin North Am. 2022 Feb;40(1):33-37. doi: 10.1016/j.emc.2021.08.008. Epub 2021 Nov 2.ABSTRACTAfter treating the acute anaphylactic reaction, the clinician's next task is to prevent a recurrence. The patient should be observed in the ED. How long this observation period should last depends on their clinical course, risk factors, and social support. All patients should be discharged with a prescription for 2 epinephrine autoinjectors and counseled on appropriate use. The patient should also receive education on the signs and symptoms of anaphylaxis and avoiding triggers. The patient should follow-up with an allergy...
