Chlorhexidine Skin Allergy Among Patients and Healthcare Workers in a Dialysis Setting
Chlorhexidine is an antimicrobial agent used in the dialysis setting for disinfection of skin and fistula/catheter exit sites. Allergic reactions to chlorhexidine have been reported; recently, there have been several case reports of anaphylaxis from chlorhexidine-impregnated central lines in dialysis patients undergoing renal transplantation. Dialysis patients and healthcare workers may be at risk for chlorhexidine sensitization.

Acquired cow ’s milk sensitization after liver transplant in an adult: “clinical implications” and future strategies
Identifying the mechanisms responsible for the development of food allergy in liver transplant recipients is more complex as there are several different clinical scenarios related to the immunological function...

Clinical aspects of STAT3 gain-of-function germline mutations: A Systematic Review
ConclusionsSTAT3 GOF syndrome is a new clinical entity to consider when confronted with a patient with early-onset poly-autoimmunity, lymphoproliferation, and growth failure. At this time, precise therapeutic guidelines are lacking, but use of anti-IL-6 receptor and JAK inhibitor biologics are attractive possibilities.

Hematopoietic Stem Cell Transplantation Beyond Severe Combined Immunodeficiency: Seeking a Cure for Primary Immunodeficiency
Publication date: March 2019Source: The Journal of Allergy and Clinical Immunology: In Practice, Volume 7, Issue 3Author(s): Richard MitchellHematopoietic stem cell transplantation (HSCT) can provide definitive therapy for patients with primary immunodeficiency disease (PIDD). Modern HSCT techniques and supportive care have significantly improved outcomes for patients with PIDD. This review examines current HSCT practice for PIDD other than severe combined immunodeficiency, and explores indications, risks, and long-term outcomes for this group of challenging diseases.

Outcomes and treatment strategies for autoimmunity and hyperinflammation in patients with RAG deficiency
ConclusionsAutoimmunity/hyperinflammation can be a presenting sign of RAG deficiency and should prompt further evaluation. Multi-lineage cytopenias are often refractory to immunosuppressive treatment and may require hematopoietic cell transplantation for definitive management.

Clinical, immunologic, and molecular spectrum of patients with LPS-responsive beige-like anchor protein (LRBA) deficiency: a systematic review
ConclusionsAutoimmune disorders are the main clinical manifestations of LRBA deficiency. Therefore, LRBA deficiency should be included in the list of monogenic autoimmune diseases and screening for LRBA mutations should be routinely performed for patients with these conditions.

Challenges and safety of beta-lactam desensitization during extracorporeal membrane oxygenation
Extracorporeal membrane oxygenation (ECMO) is an advanced life support therapy used in patients with refractory cardiac or respiratory failure. The growth of ECMO use internationally over the past decade has been exponential.1 It has been used as a temporizing measure for progressive respiratory failure in patients with cystic fibrosis (CF) as a bridge to lung transplantation.2 Notably, CF patients have higher rates of drug allergy than the general population and are at high risk of respiratory failure requiring ECMO.

Corrigendum
With regards to an article published in the April 2018 issue entitled “T-cell receptor αβ+ and CD19+ cell-depleted haploidentical and mismatched hematopoietic stem cell transplantation in primary immune deficiency” (J Allergy Clin Immunol 2018;141:1417-26), the authors have reported an error. In the first paragraph of the Results section, the sentence that curre ntly reads, “The median proportion of remaining TCRαβ T cells in the product after depletion was 0.28% (range 0.08% to 9.7%),” should be changed to read, “In the 15 patients for whom the data are available, the median proportion of remaining TCRαβ T...

Making a diagnosis in severe cutaneous drug hypersensitivity reactions
Purpose of review Severe cutaneous adverse reactions (SCAR) are relatively uncommon but can be life-threatening. This review focuses on the nonanaphylactic (non-IgE-mediated) phenotypes of drug hypersensitivity, with specific reference to diagnosis and management of acute generalized exanthematous pustulosis (AGEP), drug reaction with eosinophilia and systemic symptoms (DRESS), Stevens–Johnson syndrome (SJS), and toxic epidermal necrolysis (TEN). Recent findings Here, we review recent guidelines on optimal supportive care as well as publications of interventional treatment for SJS/TEN, including various immunomodula...
