Distinguishing computed tomography findings in patients with empty nose syndrome
ConclusionThese findings provide evidence there are 2 objective radiographic changes (central and posterior septal mucosa) that suggest ENS patients can be discriminated based on mucosal CT findings.

Extended interval dosing of natalizumab in multiple sclerosis
Conclusions Dosing intervals up to 8 weeks 5 days did not diminish effectiveness of NTZ therapy. Further monitoring is ongoing to evaluate if the risk of PML is reduced in patients on EID.

Lack of correlation between patient reported location and severity of facial pain and radiographic burden of disease in chronic rhinosinusitis
ConclusionFacial pain in CRS is not predicted by the radiographic extent of disease. The location and severity of facial pain reported by the patient is not a reliable marker of the anatomic location and severity of sinonasal inflammation. Pain location should not necessarily be relied upon for guiding targeted therapy.

Ultrasound’s role in detecting enthesitis in psoriatic arthritis remains debatable
MIAMI – Ultrasound-detected enthesitis was associated with both destructive and bone formation lesions on radiography of peripheral and axial joints in a study of 222 patients with psoriatic arthritis. But its lack of correlation to clinically detected enthesitis in this study and in others has...

Official American Thoracic Society Clinical Practice Guidelines: Diagnostic Evaluation of Infants with Recurrent or Persistent Wheezing.
CONCLUSIONS: Although infantile wheezing is common, there is a paucity of evidence to guide clinicians in selecting diagnostic tests for recurrent or persistent wheezing. Our committee made several conditional recommendations to guide clinicians; however, additional research that measures clinical outcomes is needed to improve our confidence in the effects of various diagnostic interventions and to allow advice to be provided with greater confidence. E-062 Cerebral Aneurysm Debuting as Rupture during Diagnostic CT Angiography: An Unexpected Worst-case Scenario
We describe a case of rupture at the time of CTA, after which the patient experienced decompensation in an unsupervised waiting area.Case presentationAn asymptomatic 40 year-old woman was referred for evaluation of a left-sided unruptured carotid bifurcation aneurysm identified on workup for headaches. Upon returning from the CTA suite, she became progressively confused, and was immediatelytransferred to the emergency department. Rapid neurologicaldeterioration ensued, as her Glasgow coma scale dropped to 3 and her pupils became unreactive at 6 mm. The question of contrast allergy was initially raised. CTA revealed a previ...

Clinical significance of worsening versus stable preradiographic MRI lesions in a cohort study of persons at higher risk for knee osteoarthritis
Conclusions In persons at higher risk, worsening MRI lesion status was associated with concurrent incident radiographic osteoarthritis and subsequent persistent symptoms. These findings suggest that such lesions represent early osteoarthritis, and add support for a paradigm shift towards investigation of intervention effectiveness at this stage.

Clinical and radiographic outcome of a treat-to-target strategy using methotrexate and intra-articular glucocorticoids with or without adalimumab induction: a 2-year investigator-initiated, double-blinded, randomised, controlled trial (OPERA)
Conclusions An aggressive triamcinolone and synthetic DMARD treat-to-target strategy in eRA provided excellent 2-year clinical and radiographic disease control independent of adalimumab induction therapy. ES progression was slightly less during and following adalimumab induction therapy. Trial registration number NCT00660647.

Autosomal recessive agammaglobulinemia associated with an IGLL1 gene missense mutation
A full term, 2-month-old, white girl presented with respiratory distress and hypoxia. Results of chest radiography were consistent with bilateral upper lobe pneumonia; no organism was identified, and a full recovery was achieved after 14 days of antibiotics and supportive therapy. Because of persistent marked leukocytosis (white blood cell count, 51 –54.2 × 103/mm3) and thrombocytosis (platelet count, 455–600 × 103/mm3), bone marrow was examined; no indication of malignant transformation was noted.

Signal intensity alteration in the infrapatellar fat pad at baseline for the prediction of knee symptoms and structure in older adults: a cohort study
Conclusions IPFP signal intensity alteration at baseline was associated with knee structural abnormalities and clinical symptoms cross-sectionally and longitudinally in older adults, suggesting that it may serve as an important imaging biomarker in knee OA.

Protective effect of HLA-DRB1*13 alleles during specific phases in the development of ACPA-positive RA
Conclusions These data indicate that HLA-DRB1*13 mainly affects the onset of ACPA-positive RA in ACPA-positive non-RA individuals. In RA, HLA-DRB1*13 influences ACPA characteristics but not the disease course. This implies that therapeutic strategies aimed at emulating the HLA-DBR1*13 protective effect may be most effective in ACPA-positive healthy individuals at risk for RA.

Ochronosis
A 56-year-old gentleman had a history suggestive of inflammatory arthritis with knee joint swelling with hip and shoulder pain for 12 years. Axial symptoms of back pain and gradually developing immobility were also present. He had received a diagnosis of ankylosing spondylitis, and non-steroidal anti-inflammatory drugs with disease modifying antirheumatic drugs had been tried without much relief. He underwent surgical replacement of the knees 8 months ago, when the surgeon noticed that the cartilage was black in colour. No one in the family was similarly affected. Examination showed scleral hyperpigmented lesions...

Rare-26. gliomatosis, to be or not to cerebri: an evaluation of host systemic immune factors that potentially predispose to diffuse infiltrative glioma patterns
Gliomatosis cerebri (GC) has been debated as a pathologic entity sui generis versus a simple imaging and histologic pattern. Whereas the World Health Organization (WHO) 2000 classification considered GC a distinct tumor entity, WHO 2007 downgraded GC to a diffuse glioma pattern of extensive CNS infiltration, mostly of astrocytic phenotype involving at least three cerebral lobes, and the WHO 2016 further downgraded GC by removing the chapter. Tumors with distinctive GC presentation have been proven to be molecularly heterogeneous; however, paucity of data defining the pathophysiology and genetic driver(s) of this extensive ...
