A 60-year-old woman with debilitating gout experienced HLA-B*58:01-restricted allopurinol drug reaction with eosinophilia and systemic symptoms (DRESS) syndrome. Two years later, following therapeutic failure with febuxostat, pegloticase was trialed. Twelve days following initial infusion, she developed angioedema and a diffuse erythematous pruritic rash. She self-treated with diphenhydramine, but symptoms persisted for two days. She then developed shortness of breath and throat constriction, requiring antihistamines and systemic steroids from an outside emergency department.

Anaphylaxis to a particular brew of beer due to specific strains of Saccharomyces cerevisiae
A 33 year-old male was referred to our outpatient allergy clinic after two separate instances of allergic reactions following consumption of commercial artisanal beer. The first instance was over a decade prior to presenting, but the patient had minimal recollection of the occurrence. The second instance, symptom onset occurred after two sips. He noticed immediate difficulty breathing, throat tightness chest tightness, and dyspnea. He self-administered diphenhydramine and symptoms resolved by the time he presented to the emergency.

A Case of Massive Diphenhydramine and Naproxen Overdose
Diphenhydramine, a first generation H1 histamine receptor antagonist, is a commonly used nonprescription medication that is used for the treatment of allergy, as a sleep aid, or combined with cough and cold remedies. Naproxen, a nonsteroidal anti-inflammatory drug (NSAID), is used commonly for analgesia. Although most cases of diphenhydramine or naproxen overdose require excellent supportive care only, meticulous attention should be given to cardiovascular and neurologic status.
