High oral corticosteroid exposure and overuse of short-acting beta-2-agonists were associated with insufficient prescribing of controller medication: a nationwide electronic prescribing and dispensing database analysis
ConclusionsExposure to high-dose of OCS (101 per 100,000 patients) and SABA over-use (24 per 100,000) were frequent, and were associated with a low maintenance-to-total prescription ratio but not with primary non-adherence. These results suggest there is a need for initiatives to reduce OCS and SABA inappropriate prescribing.

M063 oral budesonide desensitization: a case report
The incidence of glucocorticoid hypersensitivity reaction is approximately 0.1%. Among various glucocorticoids, the incidence of hypersensitivity appears to be highest for prednisone, prednisolone, hydrocortisone, and methylprednisolone. Budesonide is generally well tolerated. Budesonide has been reported to cause delayed-type hypersensitivity reactions, however, there have not been many reports of immediate-type hypersensitivity reactions. There are no published protocols for budesonide desensitization.

Retrospective study on the combined use of voriconazole and prednisolone in the treatment of acute allergic bronchopulmonary aspergillosis (ABPA)
Three years ago we started treating patients diagnosed with ABPA, an allergic lung disease provoked by Aspergillus fumigatus (Af), with a combined therapy of voriconazole and prednisolone. We used voriconazole at a dose of 200 mg twice a day for 4 months, while the initial prednisolone dose was 0.75 mg/kg/day for 1 month, then 0.25 mg/kg/day for 1 month, then progressive tapering (5 mg each fortnight) until the minimum dose of 5 mg/day, maintained until the fourth month. Low prevalence of obstructive sleep apnoea in paediatric severe therapy resistant asthma
Discussion: The prevalence and severity of OSA in our cohort of STRA children was lower than published in unselected asthmatics. This may reflect their proven good adherence to high dose anti-inflammatory medication at the time of their sleep study.

Late Breaking Abstract - Matrix metalloproteinases in serum and sputum reflect distinct processes of relevance to asthma
Conclusion: As local airway MMPs emerge as biomarkers of novel asthma phenotypes we find that serum and sputum MMPs differ regarding associations with disease severity and steroid use. Serum MMP-3 stands out as being strongly upregulated by steroid use, questioning its usefulness and highlighting a need to determine therapeutic effects on new biomarkers.

The impact of depression in patients with severe asthma (SA) in the Wessex Severe Asthma Cohort (WSAC)
Discussion: SA patients with depression report poorer disease control & QOL associated with a greater burden of co-morbidities including obesity, rhinosinusitis & reflux. However, no statistically significant differences were identified in their other clinical or physiological characteristics. Inflammatory phenotypes did not differ between the groups but those with depression were found to have significantly higher serum hs-CRP & sputum IL-6 levels (not seen when subdivided for BMI). Treatment of naturally occurring asthma with inhaled fluticasone or oral prednisolone: A randomized pilot trial.
The objective of this study was to compare inhaled glucocorticoids with oral glucocorticoids for treatment of naturally occurring feline asthma. Secondary goals were to evaluate serum allergy testing results in cats and to quantify the effect of an inhaled glucocorticoid (fluticasone) on glucose homeostasis. Nine cats with asthma were enrolled on the basis of clinical signs, thoracic radiographic findings, and airway eosinophilia.
