Acupuncture in the emergency department: a systematic review of randomised controlled trials.
CONCLUSION: There is a lack of high-quality evidence to support the use of acupuncture in the ED. Multicentre RCTs with rigorous designs are warranted. PMID: 29581138 [PubMed - as supplied by publisher]

Non-invasive patient-controlled analgesia in the management of acute postoperative pain in the hospital setting.
CONCLUSIONS: Evidence-based guidelines for acute postoperative pain management support the use of multimodal regimens in many situations. As effective and safe alternatives to conventional PCA, and with the added benefits of being non-invasive, easy to use and allowing early patient mobilization, the newer PCA systems may complement multimodal approaches, or potentially replace certain regimens, in hospitalized patients with acute postoperative pain. Opioids and Cardiac Arrhythmia: A Literature Review
Conclusion: The cardiac arrhythmogenicity of various opioids is different. Methadone has a higher capability to induce long QT interval and dangerous arrhythmias in conventional doses than others. To reduce of arrhythmogenic risk, high doses of opioids must be used cautiously with periodic monitoring of electrocardiogram in high-risk consumers such as patients under opioid maintenance treatment.

Development of an Institutional Opioid Prescriber Education Program and Opioid-Prescribing Guidelines: Impact on Prescribing Practices.
CONCLUSIONS: We developed a prescriber education program and followed up with consensus-based guidelines for postoperative opioid prescriptions. These interventions caused a significant decrease in excessive opioid-prescribing practices after ambulatory orthopaedic surgery at our hospital. We urge initiatives by national orthopaedic organizations to develop and promote education programs and procedure and disease-specific opioid-prescribing guidelines. Biased agonism as an emerging strategy in the search for better opioid analgesics.
Abstract Morphine and related drugs that act through activating opioid receptors are the most effective analgesics for the relief of severe pain. They have been used for decades, despite the range of unwanted side effects that they produce, as no alternative has been found so far. The major goal of opioid research is to understand the mechanism of action of opioid receptor agonists and to improve therapeutic utility of opioid drugs. In the search for safer and more potent analgesics, analogs with mixed opioid receptor profile gained a lot of interest. However, recently the concept of biased agonism, that highlight...

Overdose Deaths Far Outpace COVID-19 Deaths in San Francisco (CA)
SAN FRANCISCO (AP) — A record 621 people died of drug overdoses in San Francisco so far this year, a staggering number that far outpaces the 173 deaths from COVID-19 the city has seen thus far. The crisis fueled by the powerful painkiller fentanyl could have been far worse if it wasn’t for the nearly 3,000 times Narcan was used from January to the beginning of November to save someone from the brink of death, the San Francisco Chronicle reported Saturday. Consideration of immunomodulatory actions of morphine in COVID-19 - Short report.
Authors: Hudzik B, Nowak J, Zubelewicz-Szkodzinska B Abstract Cytokine storm in COVID-19 is linked to disease severity and mortality. 40% of patients with severe COVID-19 require mechanical ventilation. Analgesia and sedation are used for treatment of pain, facilitation of mechanical ventilation, or management of acute agitation. Herein, we present the immunomodulating actions of morphine that may either improve or worsen the clinical course of COVID-19 once cytokine storm develops. A literature search was performed to find articles on potential immunomodulatory effects of morphine. Taken together, the results of i...

Psychosis after buprenorphine, heroin, methadone, morphine, oxycodone, and tramadol withdrawal: a systematic review
CONCLUSIONS: This study encompasses a comprehensive description of the literature concerning the possible not well-studied outcome of opioid withdrawal. There are some reports of temporal association between withdrawal and psychotic symptoms that improved after the reintroduction of the opioid; it could be taken into consideration in the clinical practice.PMID:34286498 | DOI:10.26355/eurrev_202107_26248
