Intracerebral haemorrhage in normotensive patient with probable neurosaroidosis
Conclusion Vasculopathy in neurosarcoidosis results from granulomatous destruction of vessel walls, perivascular inflammation and/or steroid-induced collagen fragility. This case highlights an under-recognised and serious complication that may occur as a result. Aggressive disease suppression via immunomodulatory treatment may help minimise the risk of haemorrhagic events.

Neuromyelitis optica spectrum disorder: a decade to get a diagnosis that fits
We describe a case of an NMOSD relapse in which seizures were the main clinical feature. A twenty three year old female first presented in 2006, then thirteen with hyponatraemia and bilateral, severe optic neuritis, in the context of hypothalamic and optic chiasm lesions on MRI brain. Central pontine myelinosis was initially diagnosed but this was revised to an acute disseminated encephalomyelitis (ADEM) in 2013. We present a unique case of paraneoplastic Sweet's syndrome associated with a primary CNS lymphoma. A 69-year old gentleman presented with pyrexia of unknown origin (PUO), Sweet's syndrome and marked neutrophilia. 3 repeated bone marrow aspirates demonstrated a myelodysplastic-like syndrome. Initial treatment with hydroxycarbamide ameliorated the PUO and skin lesions. He subsequently presented with a clinical syndrome mimicking motor neurone disease; developing dysphagia, weakness and weight loss. Case report: pandysautonomia
We present the case of a 25 year old previously healthy female who became unwell with abdominal pain and vomiting a day after taking part in a half marathon and was found to have a pseudo-obstruction requiring laparotomy. Post surgery she developed a dry mouth and urinary retention and was consistently tachycardic with labile blood pressure. She developed one episode of parotitis which resolved with antibiotics. Norse associated with anti-glycine receptor antibodies
Discussion GlyR-Abs are typically associated with progressive encephalitis with rigidity and myoclonus (PERM), but have been reported to cause epileptic encephalopathies. Experience with our patient and reports in the literature suggest that GlyR-Abs cause a monophasic epileptic encephalopathy. Based on data from a small number of published cases, it may be possible to induce prolonged remission with pulsed methylprednisolone, without prolonged immunosuppression.

Neuromyelitis optica in hiv: a case report and literature review

A 49 year old lady of African descent with known HIV diagnosed in 2004 – compliant with her antiretroviral medication, Atripla – was admitted with a 3 week history of thoracic back pain, spreading paraesthesia and weakness involving both legs, and constipation. S7. neosensitization to multiple drugs following drug reaction with eosinophilia and systemic symptoms syndrome (dress)
DiscussionTo our knowledge, this is the first case report of neosensitization to multiple drugs after valproate-induced DRESS syndrome. A thorough search of Pubmed was performed to identify similar cases, which confirmed that no cases of hypersensitivity to amoxicillin or neosensitization to multiple drugs after a valproate-related DRESS episode have been reported. Furthermore, only two studies have reported possible neosensitization to amoxicillin following DRESS episodes induced by carbamazepine, and only one case reported neosensitization to amoxicillin following a DRESS episode induced by allopurinol.
