Cryptogenic stroke in a young patient with heart disease and kidney failure.
CONCLUSIONS: Fabry's disease must be suspected in young males with heart disease, stroke or peripheral neuropathy, skin lesions, kidney failure and a history of cases in the family. Hormone replacement therapy must be established at an early stage, as it can improve the prognosis. PMID: 28497441 [PubMed - in process]

Visual field defect after ischemic stroke —impact on mortality
ConclusionsHaving a visual field defect after ischemic stroke is independently associated with increased mortality. This should be addressed when selecting candidates for thrombolysis and in the rehabilitation process.

Glomerular Filtration Rate is Associated with Hemorrhagic Transformation in Acute Ischemic Stroke Patients without Thrombolytic Therapy.
Conclusions: The present study strongly showed that lower GFR is an independently predictor of HT; in addition, large infarct volume, AF, and HA are also important risks of HT for AIS patients without TT, which offered a practical information that risk factors should be paid attention or eliminated to prevent HT for stroke patients though the level of evidence seems to be unstable. Vitamin K Antagonist–associated Intracerebral Hemorrhage: Lessons from a Devastating Disease in the Dawn of the New Oral Anticoagulants
Conclusions: VKA-associated ICH presents in a particularly vulnerable population and has a poor prognosis that may be reliably predicted by several clinicoradiologic features.

Association Between Left Atrial Abnormality on ECG and Vascular Brain Injury on MRI in the Cardiovascular Health Study Clinical Sciences
Conclusions— ECG left atrial abnormality is associated with vascular brain injury in the absence of documented atrial fibrillation.

Thrombus composition in acute ischemic stroke: A histopathological study of thrombus extracted by endovascular retrieval
Conclusion HAS was significantly associated with early phase thrombus composition. This may enable the prediction of thrombus composition and allow for targeted selection of therapeutic modality.

Stroke prevention following modified endoscopic ablation and appendectomy for atrial fibrillation
We reported the results of stroke prevention following modified endoscopic procedure for atrial fibrillation. 82 patients underwent modified endoscopic procedure for atrial fibrillation (AF), in whom 47 had paroxysmal, 28 had persistent, and 7 had long-standing atrial fibrillation. CHA2DS2VASC median score was 3 (range from 0 to 8). The procedure was performed on the beating heart, through 3 ports on the left chest wall. Pulmonary vein isolation and ablation of the left atrium were achieved by bipolar radiofrequency ablation. Left atrial appendage (LAA) was excluded by stapler. Brain CT, cardiac CT and 24-h Holter monitori...

How common is isolated dysphasia among patients with stroke treated with intravenous thrombolysis, and what is their outcome? Results from the SITS-ISTR
Conclusions A low proportion of patients with isolated dysphasia are treated with IVT. Half of these patients were fully recovered at 7 days.

Midbrain and bilateral paramedian thalamic stroke due to artery of Percheron occlusion
Conclusion Bilateral thalamic stroke due to artery of Percheron occlusion is a rare presentation of stroke, which can be overlooked in routine CT scan. If diagnosed, it requires further evaluation for stroke risk factors, especially cardiovascular disorders associated with increased embolic risk.

Right-side propensity of cardiogenic emboli in acute ischemic stroke with atrial fibrillation.
CONCLUSION: This study demonstrated the existence of a right-side propensity of cardiogenic emboli and the larger infarct volume of right-side MCA/ACA stroke in patients with AF. These results can be attributed to anatomical differences between the innominate and the left common carotid artery. PMID: 25375813 [PubMed - indexed for MEDLINE]

Early introduction of direct oral anticoagulants in cardioembolic stroke patients with non-valvular atrial fibrillation
AbstractDirect oral anticoagulants (DOACs) are superior to warfarin in reduction of the intracranial bleeding risk. The aim of the present study was to assess whether early DOAC introduction (1 –3 days after onset) in stroke patients with non-valvular atrial fibrillation (nVAF) may be safe and effective, compared with DOAC introduction after 4–7 days. We conducted a prospective analysis based on data collected from 147 consecutive nVAF patients who started DOAC within 7 days after stro ke onset. In all patients, we performed pre-DOAC CT scan 24–36 h after onset and follow-up CT scan at 7 days after DOAC introduction...

Prehospital pathways of occipital stroke patients with mainly visual symptoms
ConclusionsOccipital stroke patients with visual symptoms contact health care late, are inadequately recognized, and present with complex prehospital pathways. Consequently, they are often ineligible for IV thrombolysis. This presents a missed opportunity for preventing permanent visual field defects.

Predictors of hematoma volume in the patients treated with intravenous alteplase (P4.297)
CONCLUSIONS: Hematoma volume is related with short and long-term clinical outcomes. Atrial fibrillation and early CT changes are independent predictors of large hematoma volume.Disclosure: Dr. Heo has nothing to disclose. Dr. Lee has nothing to disclose. Septic Thromboembolic Stroke via Atrial-Esophageal Fistula as a Delayed Complication of Cardiac Ablation (P6.018)
Conclusions: In the perioperative period following CA, diagnostic testing to rule out the often fatal complication of an A-E fistula may be non-revealing. A high index of suspicion should nonetheless be maintained that an occult A-E fistula is a cause of delayed presentation of septic cerebral thromboembolism.Disclosure: Dr. Derani has nothing to disclose.
