2-1-2 stroke microsurgical thrombectomy technique as a tertiary/salvage treatment option
CONCLUSIONS: 2-1-2 MST technique can be rapidly and efficaciously performed in a systemized manner offering a tertiary or salvage technique for acute stroke treatment. Specific microsurgical training is mandatory to accomplish this treatment and larger studies are necessary to confirm our hypothesis.PMID:35147401 | DOI:10.23736/S0390-5616.21.05480-1

O-061 Predictors of failed mechanical thrombectomy for large vessel occlusion: insights from the stroke thrombectomy and aneurysm registry (STAR)
ConclusionIn one of the largest studies to evaluate factors associated with failure MT in real world practice, we demonstrate that MTF is associated with significantly more complications and worse outcome. Final use of aspiration and rescue intracranial stenting may increase chances of recanalization.Disclosures M. Webb: None. M. Essibayi: None. S. Al Kasab: None. I. Maier: None. M. Psychogios: None. Cost-effectiveness of thrombectomy in patients with minor stroke and large vessel occlusion: effect of thrombus location on cost-effectiveness and outcomes
Conclusions EVT is cost-effective in patients with minor stroke and LVO in the long term (lifetime horizon), considering the poor outcomes and significant disability associated with non-reperfusion. Our study emphasizes the need for caution in interpreting previous observational studies which concluded similar results in EVT versus medical management in patients with minor stroke due to a high proportion of patients with M2 occlusions in the two strategies.

Comparative analysis between 1-D, 2-D and 3-D carotid web quantification
Conclusion CaW dimension quantification (1-D, 2-D and 3-D) is highly reproducible. Linear and volumetric measurements were more strongly associated with symptoms. The impact of CaW size on the presence of LVOS, clot burden and final infarct volume is unclear.

E-108 Endovascular treatment of patients with acute ischemic stroke due to underlying arterial dissection: a nationwide analysis
ConclusionsDespite a similar prevalence among patients admitted for AIS, CA dissections are associated with a higher rate of endovascular intervention and poorer short-term outcomes compared to VA dissections. This study provides essential epidemiologic and outcome data for the cerebrovascular neurosurgeon.Abstract E-108 Table 1Comparison of clinical characteristics and outcomes among patients presenting with acute ischemic stroke due to carotid or vertebral artery dissections Characteristic Vertebral Artery Dissection, N = 2,562 Carotid Artery Dissection, N = 2,789 p-value1 Age (years), Mean (SD) 53 (17) 57 (17) <0.00...

Why, for once, is the UK leading the way in implementing evidence?
One of the major factors influencing the quality of healthcare is the responsiveness of clinicians to new evidence and their willingness to implement findings into routine clinical practice in a timely way.1 The factors that affect the efficiency of getting evidence into practice are complex; a good example of this is illustrated in the paper by Ankolekar et al.2 The Clots in Legs or sTockings after Stroke (CLOTS) 1 trial definitively showed that compression stockings after stroke did not prevent venous thromboembolism. The trial was well conducted and adequately powered for the primary outcome, so there was little room fo...

Thrombolytic therapy in total mismatch with severe stroke after acute MCA-occlusion and negative DWI
Total mismatch is a recently described magnetic resonance imaging (MRI) pattern in acute stroke patients with no signal changes in diffusion weighted images (DWI) and extensive hypoperfusion on perfusion weighted images (PWI). The prognostic value and importance for treatment decision based on this finding in MRI have recently been controversially discussed in patients with minor stroke . Here we report a case of a 98-year-old woman that presented with severe stroke and total mismatch in the left middle cerebral artery (MCA) territory in MRI scan and an occlusion of the proximal MCA (M1) in time-of flight MR angiography (T...

Last resort: case of clot translocation in intra-arterial stroke therapy
A patient was taken for emergent intra-arterial stroke therapy for an acute left middle cerebral artery stroke syndrome, with CT angiography showing a left internal carotid artery (ICA) occlusion. Through a 6 F Neuron MAX sheath, a 5 Max ACE Penumbra aspiration catheter was advanced to the thrombus and direct suction was performed through the ACE catheter and Neuron MAX sheath. Upon pull back, the thrombus became wedged in the Neuron MAX sheath and despite several attempts to aspirate the thrombus, no clot could be obtained. The Neuron MAX sheath was withdrawn to the left common carotid artery, and gently advanced to the o...

Embolectomy for stroke with emergent large vessel occlusion (ELVO): report of the Standards and Guidelines Committee of the Society of NeuroInterventional Surgery
Introduction Stroke is the leading cause of adult disability in North America and is the fifth most common cause of death.1 2 The natural history of patients with acute ischemic stroke and occlusion of a major intracranial vessel such as the internal carotid artery (ICA), middle cerebral artery (MCA), or basilar artery is dismal, with high rates of mortality and low rates of disability-free survival.3 4 We introduce the term ‘Emergent Large Vessel Occlusion (ELVO)’ to describe this clinical scenario. Among acute ischemic stroke, ELVO accounts for the greatest proportion of patients with long-term disability. Fo...

Wake-up stroke in a young woman with rotational vertebral artery occlusion due to far-lateral cervical disc herniation.
Abstract Wake-up stroke is most likely to be caused by small-vessel disease, and is related to snoring. The authors present a rare case of far-lateral cervical disc herniation with neck rotation, resulting in wake-up stroke in a young woman. The patient, a 31-year-old woman, was admitted to the hospital because of dysarthria and confusion when she awoke in the morning. Brain MRI showed acute infarction in the posterior fossa. Cerebral angiography showed thrombus in the distal top of the basilar artery and the bilateral posterior cerebral arteries. During angiography, the thrombus size decreased with heparinization...

P-013 initial experience with the ace and ace64 cerebral reperfusion catheters in the endovascular treatment of acute ischemic stroke
ConclusionIn our initial experience, the ACE/ACE64 reperfusion catheters provide a safe and effective tool for the treatment of acute ischemic stroke using direct aspiration, in which we were able to recanalize (TICI 2B/3) 96.4% of cases, achieving good clinical results.DisclosuresP. Is acute stress and plasminogen activator inhibitor polymorphism associated with acute ischemic stroke?
Identifying the cause of ischemic stroke (IS) in young adults is of major importance in preventing stroke recurrence. In most cases, IS is a multifactorial disease with genetic predisposition, whereas the inherited risk is likely to be multigenic. Alterations in thrombosis and fibrinolysis comprise important parts of stroke pathophysiology. Plasminogen activator inhibitor type-1 (PAI-1) is an important component of the coagulation system that is a strong inhibitor of fibrinolysis in the circulation.

Midbrain and bilateral paramedian thalamic stroke due to artery of Percheron occlusion
Conclusion Bilateral thalamic stroke due to artery of Percheron occlusion is a rare presentation of stroke, which can be overlooked in routine CT scan. If diagnosed, it requires further evaluation for stroke risk factors, especially cardiovascular disorders associated with increased embolic risk.

Middle cerebral artery fusiform aneurysm presented with stroke and delayed subarachnoid hemorrhage trapping, thrombectomy, and bypass.
CONCLUSION: Our case describes a complex aneurysm treatment that consisted of aneurysm trapping, thrombus removal and an STA-M3 MCA branch bypass creation for the protection of the patent M3 insular MCA branch and prevention of further ischemia. This procedure rewarded us with an excellent clinical result. PMID: 27127709 [PubMed]
