ASTRAL-R score predicts non-recanalisation after intravenous thrombolysis in acute ischaemic stroke.
In conclusion, the 5-item ASTRAL-R score moderately predicts non-recanalisation at 24 hours in thrombolysed ischaemic strokes. If its performance can be confirmed by external validation and its clinical usefulness can be proven, the score may influence patient selection for more aggressive revascularisation strategies in routine clinical practice. 64-detector CT angiography within 24 hours after carotid endarterectomy and correlation with postoperative stroke.
CONCLUSIONS CTA within 24 hours of CEA demonstrates characteristic anatomical findings. CCA step-offs and ECA flaps are relatively common and clinically insignificant, whereas ICA/CCA flaps and thrombi are less frequently seen and are associated with postoperative stroke/TIA. Coronary Stent Thrombosis With Vorapaxar Versus Placebo Results From the TRA 2°P-TIMI 50 Trial
BackgroundVorapaxar, a novel thrombin receptor antagonist, reduces cardiovascular death and recurrent thrombotic events when added to standard antiplatelet therapy in patients with stable atherosclerotic vascular disease.ObjectivesThe goal of this study was to test the hypothesis that treatment with vorapaxar reduces the rate of coronary stent thrombosis (ST) in stable patients with a history of coronary stenting.MethodsTRA 2°P-TIMI 50 (Trial to Assess the Effects of Vorapaxar in Preventing Heart Attack and Stroke in Patients With Atherosclerosis-Thrombolysis In Myocardial Infarction 50) was a multinational, randomized, d...

Postoperative warfarin following mitral valve repair or bioprosthetic valve replacement.
CONCLUSION: The use of postoperative warfarin following MVR does not reduce the incidence of stroke at early follow up. However, there remains a trend for improved long-term outcomes in those patients receiving postoperative warfarin therapy. PMID: 24383387 [PubMed - indexed for MEDLINE]

The Significance of Preoperative Impaired Sensorium on Surgical Outcomes in Nonemergent General Surgical Operations
ImportanceWith an aging population, preoperative assessment of the frail older adult requires evaluation beyond simply accounting for chronic diseases. Impaired cognition is a recognized characteristic of the frail older adult.ObjectiveTo examine the effect of preoperative impaired sensorium (IS) on general surgical outcomes.Design, Setting, and ParticipantsRetrospective cohort study using data between January 2005 and December 2010 at academic and community hospitals participating in the American College of Surgeons National Surgical Quality Improvement Program. Patients undergoing nonemergent general surgical operations ...

Second-Generation Drug-Eluting Stent Implantation Followed by 6- Versus 12-Month Dual Antiplatelet Therapy The SECURITY Randomized Clinical Trial
BackgroundThe optimal duration of dual antiplatelet therapy (DAPT) following second-generation drug-eluting stent (DES) implantation is still debated.ObjectivesThe aim of this study was to test the noninferiority of 6 versus 12 months of DAPT in patients undergoing percutaneous coronary intervention with second-generation DES.MethodsThe SECURITY (Second Generation Drug-Eluting Stent Implantation Followed by Six- Versus Twelve-Month Dual Antiplatelet Therapy) trial was a 1:1 randomized, multicenter, international, investigator-driven, noninferiority study conducted from July 2009 to June 2014. Patients with a stable or unst...

Computed Tomography Abnormalities and Epidemiology of Adult Patients Presenting With First Seizure to the Emergency Department in Qatar / Epidemiología y Alteraciones en la Tomografía Computarizada de los Pacientes Adultos Atendidos por una Primera Crisis Epiléptica en un Servicio de Urgencias en Qatar.
ConclusionsAdults who presented with first seizure to the ED in Qatar had a young male predominance, and a high proportion of brain CT scans were reported as abnormal. It is recommended that all such patients in this population should undergo prompt CT scanning in the ED, but the utility of routine electrolyte tests requires further investigation.

Bare-Metal vs. Drug-Eluting Stents in Patients With Atrial Fibrillation Undergoing Percutaneous Coronary Intervention.
Conclusions:In real-world patients with AF undergoing PCI, DES use was associated with outcomes comparable to those with BMS without excess bleeding complications. More ST was seen in BMS-treated patients. PMID: 25298168 [PubMed - as supplied by publisher]

Perioperative management of oral antiplatelet therapy and clinical outcomes in coronary stent patients undergoing surgery. Results of a multicentre registry.
In conclusion antiplatelet discontinuation increases the 30-day risk of MACE, in patients with coronary stents undergoing cardiac and non-cardiac surgery, while not offering significant protection from BARC≥ 2 bleeding. PMID: 25274620 [PubMed - as supplied by publisher]

Reducing Time to Treatment - The Role of Stroke Neurologist as First Responder (S5.006)

Utility of Workup for Thrombophilias in a Single Center - A Qualitative Perspective (P1.125)
ConclusionStroke is considered cryptogenic in about 30-40 percent cases. Hypercoagulable testing is often ordered in this clinical setting. So far our results are consistent with other reports in the literature that such testing has limited clinical utility. Our results highlight the need to develop stringent clinical guidelines for HT in the setting of ischemic strokes.Disclosure: Dr. Shah has nothing to disclose. Dr. Anderson has nothing to disclose. Utility of Pelvic MRV in Evaluation of Patients with Cryptogenic Stroke and PFO (P4.228)
CONCLUSIONS: Our data expands on prior study showing that pelvic MRV has the potential to identify a stroke source in patients with cryptogenic stroke, absent identifiable hypercoagulability state, and PFO.Disclosure: Dr. Osgood has nothing to disclose.
