This report is the first case of cervical epidural hematoma in a healthy donor who underwent peripheral blood stem cell apheresis and presented symptoms confusingly similar to those of brain infarction.

Upper Extremity Ischemia as a “Warning Shot” of Cerebellar Infarction
A 44-year-old woman without cardiovascular risk factors complained of pain and coldness in the digits of her left hand. Radial and ulnar pulses were palpable, with equal brachial pressures on both sides. After a 2-hour stay in the outpatient department she developed acute headache, nausea, and vomiting. Computed tomographic scan with contrast medium revealed a floating thrombus in the left subclavian artery extending towards the origin of the vertebral artery (A). Brain magnetic resonance imaging demonstrated an infarction of the left cerebellum (B), whereupon the patient was immediately referred to the stroke unit.

Skew Deviation: Case Report and Review of the Literature.
CONCLUSION: Skew deviation, a not uncommon clinical condition, should be promptly recognized when binocular vertical diplopia cannot be interpreted by trochlearis and oculomotor nerve lesion, myasthenia gravis, or orbital pathology. Maddox rod, cover test, Parks-Bielschowsky three-step, and other tests should help to establish the diagnosis. The prognosis depends on etiology, but it is commonly favorable; the majority of patients recover spontaneously after less than a year. Dermatomal Sensory Manifestations in Opalski Syndrome
A 31-year-old Japanese woman presented with sudden-onset unstable gait followed by nuchal pain. A neurological examination revealed right-sided limb weakness and decreased pain and thermal sensation on the left side below the level of the L1 dermatome. A lower lateral medullary infarction with ipsilateral hemiplegia, known as Opalski syndrome, caused by spontaneous vertebral artery dissection was diagnosed by magnetic resonance imaging. The spinothalamic tract in the medulla oblongata has a topographic arrangement of sensory fibers, and the dermatomal sensory deficit in this case can be explained in relation to that.

Cervical artery dissections (CeAD) include both internal carotid and vertebral artery dissections. They are rare but important causes of stroke, especially in younger patients. CeAD should be considered in patients with strokelike symptoms, a new-onset headache and/or neck pain, and/or other risk factors. Early imaging with computed tomography (CT) or magnetic resonance imaging (MRI) is key to making the diagnosis. Treatment may vary depending on the extent of the dissection, timing of the dissection, and other comorbidities. The overall prognosis is good, but does depend on the initial severity of symptoms.

Improvement and Aggravation of Spontaneous Unruptured Vertebral Artery Dissection
Conclusions: VAD improvement primarily occurs within 6 months after onset, and VAD aggravation within 1 month. Acute Intracranial and Spinal Subdural Hematoma Associated with Vardenafil
A 28-year-old healthy man was admitted to our hospital because of right-sided headache, vomiting, and lower back pain after the administration of vardenafil. Computed tomography and magnetic resonance imaging of the brain showed a small, right-sided, subdural hematoma. A lumbar magnetic resonance imaging showed a longitudinally extended subdural hematoma. He had no history of trauma. We speculated that vardenafil might have had an association with the bleeding. Several reports have suggested a relationship between phosphodiesterase-5 inhibitors and intracerebral or subarachnoid hemorrhage.
