Starry Eye
A 73-year-old female with a past medical history of stroke, hypertension, end stage renal disease on hemodialysis presents to the emergency department for altered mental status and slurred speech. On physical examination, she had a right pupillary defect in a star-like pattern with initial concern for a blown pupil (see image) raising suspicion for acute stroke. Her initial blood pressure was 230/110 mmHg and neuroimaging were unremarkable for stroke. She was admitted for hypertensive emergency.

Prevalence of hypertension and dyslipidemia and their associations with micro- and macrovascular diseases in patients with diabetes in Taiwan: An analysis of nationwide data for 2000-2009.
CONCLUSION: Although progressively increased prevalence of dyslipidemia and hypertension was observed in patients with diabetes in Taiwan, there was a decrease in the prevalence of stroke and CVD in the past 10 years. Among those with macrovascular diseases, except PVD, there was a trend of decreased prevalence of hypertension and dyslipidemia during the study period. In patients with microvascular diseases, prevalence of hypertension and dyslipidemia in patients with eye diseases increased in the past 10 years. Home blood pressure measurement as a systematic tool for clinical practice in CKD patients: a real-world picture.
CONCLUSIONS: We observed a quite high rate of masked uncontrolled hypertension and of white coat hypertension in stage 3-5ND CKD patients. Office BP measurement, as a single tool, is an inadequate diagnostic procedure in the clinical management of CKD patients. HBPM should be routinely implemented for identifying hypertensive phenotypes and then for avoiding misdiagnosis and mistreatment of pre-dialysis CKD patients in a tertiary care setting. Hypoxia-inducible factor stabilisers for the anaemia of chronic kidney disease
CONCLUSIONS: HIF stabiliser management of anaemia had uncertain effects on CV death, fatigue, death (any cause), CV outcomes, and kidney failure compared to placebo or ESAs.
