Does stroke etiology influence outcome in the posterior circulation? An analysis of 107 consecutive acute basilar occlusion thrombectomies
CONCLUSIONS: In the current study, demographic and clinical results for acute BAO showed that compared with intrinsic disease, thromboembolic disease is a more common mechanism of acute BAO, with 78% of patients undergoing thrombectomy alone. However, there was no significant difference in revascularization and outcome results between patients with embolic disease and those with intrinsic disease.PMID:34198247 | DOI:10.3171/2021.4.FOCUS2189

The effect of occupational exposure to noise on ischaemic heart disease, stroke and hypertension: A systematic review and meta-analysis from the WHO/ILO Joint Estimates of the Work-Related Burden of Disease and Injury
CONCLUSIONS: For acquiring IHD, we judged the existing body of evidence from human data to provide "limited evidence of harmfulness"; a positive relationship is observed between exposure and outcome where chance, bias, and confounding cannot be ruled out with reasonable confidence. For all other included outcomes, the bodies of evidence were judged as "inadequate evidence of harmfulness". Producing estimates for the burden of CVD attributable to occupational exposure to noise appears to not be evidence-based at this time.PROTOCOL IDENTIFIER: 10.1016/j.envint.2018.09.040.PROSPERO REGISTRATION NUMBER: CRD42018092272.PMID:336...

State-of-the-Art review: Home blood pressure monitoring: current status and new developments.
Abstract Home blood pressure monitoring (HBPM) is a reliable, convenient and less costly alternative to ambulatory blood pressure monitoring (ABPM) for the diagnosis and management of hypertension. Recognition and use of HBPM has dramatically increased over the last 20 years and current guidelines make strong recommendations for the use of both HBPM and ABPM in patients with hypertension. The accuracy and reliability of home BP measurements requires use of a validated device and standardized procedures, and good patient information and training. Key HBPM parameters include morning BP, evening BP and the morning-ev...

The Association of Low Molecular Weight Heparin Use and In-hospital Mortality Among Patients Hospitalized with COVID-19
ConclusionsAmong hospitalized COVID-19 patients, LMWH use was associated with lower all-cause in-hospital mortality than non-LMWH users. The survival benefit was particularly significant among more severely ill patients.

Healthful dietary patterns and risk of end-stage kidney disease: the Singapore Chinese Health Study.
CONCLUSIONS: Adherence to healthful dietary patterns was associated with a lower ESKD risk in an Asian population, especially in overweight or obese individuals. PMID: 33381807 [PubMed - as supplied by publisher]

N-Terminal Pro-B-Type Natriuretic Peptide and Longitudinal Risk of Hypertension.
CONCLUSIONS: NT-proBNP was not associated with incident hypertension; this did not differ by race or sex. Cross-sectional associations of natriuretic peptides with hypertension are likely due to myocardial stretch due to increased afterload. PMID: 33378421 [PubMed - as supplied by publisher]

Acute Kidney Injury in Acute Ischemic Stroke Patients in Clinical Trials
Objectives: Acute ischemic stroke patients are at risk of acute kidney injury due to volume depletion, contrast exposure, and preexisting comorbid diseases. We determined the occurrence rate and identified predictors associated with acute kidney injury in acute ischemic stroke patients. Setting: Multiple specialized ICUs within academic medical centers. Design: Post hoc analysis of pooled data from prospective randomized clinical trials. Patients: Acute ischemic stroke patients recruited within 3 hours or within 5 hours of symptom onset. Interventions: IV recombinant tissue plasminogen activator, endovascular...

Type A aortic dissection in the East and West: A comparative study between two hospitals from China and the US
ConclusionsThere was a significant increase in the number of ATAAD at NDTH during the study period while the number at MGH remained stable. Hypertension was a common major risk factor; however, the onset of ATAAD at NDTH was nearly one decade earlier than MGH. Chinese patients tended to have more complicated preoperative pathophysiology at presentation and underwent more extensive surgical repair.

Endovascular therapy versus medical treatment for symptomatic intracranial artery stenosis.
CONCLUSIONS: This systematic review provides moderate-quality evidence showing that ET, compared with CMT, in people with recent symptomatic severe intracranial atherosclerotic stenosis probably does not prevent recurrent stroke and appears to carry an increased hazard. The impact of delayed ET intervention (more than three weeks after a qualifying event) is unclear and may warrant further study. PMID: 32789891 [PubMed - in process]

Blood Pressure Variability and Dementia: A State-of-the-art Review.
Abstract Accumulating evidence demonstrates that blood pressure variability (BPV) may contribute to target organ damage, causing coronary heart disease, stroke, and renal disease independent of the level of BP. Several lines of evidence have also linked increased BPV to a higher risk of cognitive decline and incident dementia. The estimated number of dementia cases worldwide is nearly 50 million, and this number continues to grow with increasing life expectancy. Because there is no effective treatment to modify the course of dementia, targeting modifiable vascular factors continues as a top priority for dementia p...
