Effect of tirofiban in treating patients with progressive ischemic stroke
CONCLUSIONS: Tirofiban combined with conventional basic treatment can greatly improve neurological deficits and disease outcomes, alleviate platelet adhesion, and reduce platelet activation without increasing the risk of hemorrhage in PIS patients.PMID:35363359 | DOI:10.26355/eurrev_202203_28357

Percutaneous left atrial appendage closure in patients with primary hemostasis disorders and atrial fibrillation
ConclusionsPercutaneous LAAC in primary HD carriers appeared as safe and as effective as in overall LAAC population for stroke and bleeding prevention at midterm follow-up.Graphical abstractPercutaneous left atrial appendage closure in patients with atrial fibrillation and primary hemostasis disorders. The percutaneous LAAC in primary hemostasis disorders and AF carriers requires a multidisciplinary approach. Cardiologist, anesthesiologist, and hematologist discussion is a cornerstone to assess anticoagulant contraindication, LAAC feasibility, periprocedural management, and follow-up(high). This multidisciplinary care is i...

Efficacy and safety of direct oral anticoagulants for treatment of left ventricular thrombus; a systematic review
CONCLUSION: Based on our review, DOACs are likely to be at least as effective and safer as VKA for stroke prevention and thrombus resolution in patients with LVT.PMID:33970796 | DOI:10.1080/00015385.2021.1901024

Direct Oral Anticoagulants Versus Warfarin in the Treatment of Left Ventricular Thrombus.
CONCLUSION AND RELEVANCE: Our data suggest that DOACs may be reasonable alternatives for treatment of LV thrombus. When added to the totality of available studies, this study demonstrates that the effectiveness of DOACs in LV thrombus remains uncertain. Randomized clinical trials are needed. Trends in Comorbidities and Complications Among Patients Undergoing Inpatient Spine Surgery
Study Design. Retrospective database study. Objective. We sought to identify trends in demographics, comorbidities, and postoperative complications among patients undergoing ACDF and PLF. Summary of Background Data. As demand for anterior cervical discectomy and fusion (ACDF) and posterior lumbar fusion (PLF) surgery continues to increase, it is important to understand changes in the healthcare system and patient populations undergoing these procedures. Methods. We identified 220,520 ACDF and 151,547 PLF surgeries (2006–2016; Premier Healthcare database). Annual proportions or medians were calculated for patie...

Intravenous Fat Emulsion Does Not Significantly Alter Clotting Markers in Dabigatran-Treated Blood
AbstractDabigatran etexilate is an oral direct thrombin (Factor IIa) inhibitor approved for patients with atrial fibrillation and for management of risk of deep vein thrombosis and pulmonary embolism. Dabigatran offers advantages over treatment with warfarin, including limited laboratory monitoring. It is equivalent in prevention of stroke and deep vein thrombosis with essentially equivalent complication rates. In contrast to warfarin, reversal of the anticoagulation is less well established. Idarucizumab is available for reversal, however supporting research is mixed; the agent also happens to be quite expensive making av...

National Outcomes of Surgical Embolectomy for Acute Pulmonary Embolism.
CONCLUSIONS: In this contemporary, real-world study, mortality occurred in 19.8% of patients undergoing surgical embolectomy for acute pulmonary embolism. This represents a significant improvement compared to traditional outcomes and supports the role of surgery in the multidisciplinary treatment of this high-risk condition.
