Diffuse cerebral petechial hemorrhage in an 8-year-old girl with MRSA pneumonia and sepsis
An 8-year-old girl in septic shock due to necrotizing methicillin-resistant Staphylococcus aureus (MRSA) pneumonia developed signs of end-organ damage, new right hemiplegia, and left gaze preference. Susceptibility-weighted MRI demonstrated extensive multifocal petechial hemorrhage preferentially at the gray–white matter interface due to septic microemboli (figure). Respiratory variation in carotid peak systolic velocity predicts volume responsiveness in mechanically ventilated patients with septic shock: a prospective cohort study
Conclusions ΔCDPV can be more accurate than other methods for assessing fluid responsiveness in patients with septic shock receiving lung protective mechanical ventilation. ΔCDPV also has a high correlation with SVI increase after fluid challenge.

Levosimendan Versus Dobutamine in Myocardial Injury Patients with Septic Shock: A Randomized Controlled Trial.
CONCLUSIONS Compared with dobutamine, levosimendan reduces biomarkers of myocardial injury and improves systemic hemodynamics in patients with septic shock. Impact of Dobutamine in Patients With Septic Shock: A Meta-Regression Analysis
Septic shock frequently requires vasopressor agents. Conflicting evidence exists for use of inotropes in patients with septic shock. Data from English studies on human adult septic shock patients were collected. A total of 83 studies were reviewed, while 11 studies with 21 data sets including 239 patients were pooled for meta-regression analysis. For VO2, pooled difference in means (PDM) was 0.274. Minimally Invasive Access Aortic Arch Surgery
Conclusions: Minimally invasive aortic arch surgery via partial upper sternotomy does not increase the risk of morbidity or mortality. Thus, in experienced hands, it is viable, safe, and therefore favorable and as a result should be offered to more patients.

The use of bedside echocardiography for measuring cardiac index and systemic vascular resistance in pediatric patients with septic shock
Conclusion: There was a persistently high systemic vascular resistance index in cold shock patients that influenced the stroke volume index, cardiac index, and velocity time integral. The use of echocardiograms for hemodynamic measurements is important in pediatric septic shock patients to adjust dilators, and vasopressor doses and achieve resuscitation targets in a timely manner.
