Activation of The Metabolic Master Regulator PPAR γ - A Potential PIOneering Therapy for Pulmonary Arterial Hypertension.
The objective is a data-driven approach to set the preclinical and clinical study record straight. The convincing recent clinical trial data on the lack of significant toxicity in high risk populations justify the timely conduction of clinical studies in order to achieve "repurposing" or "repositioning" of pioglitazone for the treatment of clinical PAH. Pioglitazone for prevention or delay of type 2 diabetes mellitus and its associated complications in people at risk for the development of type 2 diabetes mellitus.
CONCLUSIONS: Pioglitazone reduced or delayed the development of T2DM in people at increased risk of T2DM compared with placebo (low-certainty evidence) and compared with no intervention (moderate-certainty evidence). It is unclear whether the effect of pioglitazone is sustained once discontinued. Pioglitazone compared with metformin neither showed advantage nor disadvantage regarding the development of T2DM in people at increased risk (low-certainty evidence). The data and reporting of all-cause mortality, SAEs, micro- and macrovascular complications were generally sparse. None of the included studies reported on QoL or so...

Dose Response. 2022 Feb 23;20(1):15593258211068625. doi: 10.1177/15593258211068625. eCollection 2022 Jan-Mar.ABSTRACTHormetic-biphasic dose response relationships are reported herein for human endothelial progenitor cells involving estradiol, nicotine, the anti-diabetic agent pioglitazone, resveratrol, and progesterone. In general, these studies demonstrate the capacity of these agents to enhance EPC proliferation and angiogenesis functional applications, having a focus on repairing endothelial tissue damage due to acute injury (e.g., stroke), as well as damage from chronic conditions (e.g., atherosclerosis) and normal agi...

Orv Hetil. 2023 Jul 2;164(26):1012-1019. doi: 10.1556/650.2023.32783. Print 2023 Jul 2.ABSTRACTIn the past decade and a half, clinical diabetology has undergone enormous development. New drug classes have appeared in everyday practice (GLP1 receptor agonists, SGLT2 inhibitors), which are able to improve the outcome of cardiovascular (macrovascular) complications in diabetes within a few years or even a few months, in contrast to the drugs used in previous large, prospective studies (UKPDS, VADT). The use of thiazolidinediones (including pioglitazone) unfortunately and significantly has declined in recent years, both intern...
