Elevated Plasma D-Dimer Level Is Associated With Short-Term Risk of Ischemic Stroke in Patients With Acute Heart Failure Brief Report
We examined 721 consecutive hospitalized AHF patients with plasma D-dimer level on admission from our prospective registry between January 2013 and May 2016. The study end points were incidence of ischemic stroke during hospitalization and at 30 days after admission.Results—Of the total participants (mean age, 76 years; male, 60%; atrial fibrillation, 54%; mean left ventricular ejection fraction, 38%), in-hospital ischemic stroke occurred in 18 patients (2.5%) during a median hospitalization period of 21 days, and 30-day ischemic stroke occurred in 16 patients (2.2%). Risk Factors for Acute Heart Failure and Impact on In-Hospital Mortality after Stroke
Background: In the acute phase of stroke, some patients develop cardiac events. It could be fatal in their clinical courses. We aimed to investigate acute heart failure after stroke onset and stratify the patients by establishing a predictive model. Methods: This single-center, observational study included stroke patients diagnosed at the Department of Neurology and Neurosurgery from January 2013 to December 2014. Baseline characteristics and clinical findings on admission were analyzed for acute heart failure after stroke.

Globally, stroke is a major contributor to death and disability.1 Evidence-based care in hospital after an acute stroke ensures improved survival and reduced disability, but care quality can vary.2,3 Clinical coordinators have an essential role in improving best-practice in trauma, cancer, diabetes, and heart failure conditions.4-7 Within stroke, coordinator roles are not new; in 1970, a study outlining the ‘Stroke Team’ model was published, which included a physician, nurse-coordinator and physical therapist.

Dabigatran etexilate for secondary stroke prevention: the first year experience from a multicenter short-term registry
Discussion: Our pilot data indicate that dabigatran appears to be safe for secondary stroke prevention during the first year of implementation of this therapy. However, high cost may limit the long-term treatment of AF patients with dabigatran, leading to early discontinuation.

Calcified Amorphous Tumor Causing Shower Embolism to the Brain: A Case Report with Serial Echocardiographic and Neuroradiologic Images and a Review of the Literature
An 89-year-old woman with chronic atrial fibrillation, hypertension, chronic heart failure, and dementia was admitted to our hospital due to multiple small cerebral and cerebellar infarctions. Transthoracic echocardiogram revealed a floating calcified mass lesion arising from the endocardium of the posterior portion of the mitral annulus with mitral annular calcification. Furthermore, the mass had a heterogeneity of the echogenicity. The mass was diagnosed as a calcified amorphous tumor based on specific echocardiographic features.
