High-Risk Factors in Symptomatic Patients Undergoing Carotid Artery Stenting With Distal Protection: Buffalo Risk Assessment Scale (BRASS)
CONCLUSION: Our results suggest that CAS should be avoided in patients with multiple anatomic risk factors. High presenting National Institutes of Health Stroke Scale score and renal disease also increase the complication risk. The CAS scoring system devised here is simple, reproducible, and clinically valuable in predicting complications risk in symptomatic patients undergoing CAS. ABBREVIATIONS: BRASS, Buffalo Risk Assessment Scale CAS, carotid artery stenting CCA, common carotid artery CEA, carotid endarterectomy CTA, computed tomographic angiogram ICA, internal carotid artery MI, myocardial infarction NASCET, North Ame...

Antithrombotic therapy for atrial fibrillation and coronary artery disease in older patients
Conclusions Among older patients with AF and CAD, overall warfarin use was low. Patients ≥80 years old at highest stroke risk received warfarin in similar proportions to the overall cohort. Further investigation into optimizing antithrombotic strategies in this population is warranted.

External stenting of saphenous vein grafts for coronary artery bypass: a single-center analysis of clinical outcomes
CONCLUSIONS: Clinical outcomes with external SVG stenting are excellent without ischemia-driven targetvessel revascularization at 1 year, and low rates of repeat revascularization at 3 years. Further follow-up will show whether external stenting reduces SVG failure with a benefit on long-term clinical outcomes.PMID:35005876 | DOI:10.23736/S0021-9509.22.12008-2

Acute myocardial infarction related to coronary artery embolism: A systematic cardiac and cerebral magnetic resonance imaging study
CONCLUSION: MI-related to CAE was associated with infarctions of average size but multiple locations. Systematic brain MRI revealed that 33% of cases were associated with a stroke, which was generally asymptomatic. Further studies are required to better characterize the pathophysiology, clinical course and prognostic value of CAE. Moreover, optimal management strategies remain to be determined.PMID:35934614 | DOI:10.1016/j.acvd.2022.05.005

Fractional Flow Reserve-Guided PCI or Coronary Bypass Surgery for 3-Vessel Coronary Artery Disease: 3-Year Follow-Up of the FAME 3 Trial
CONCLUSIONS: At 3-year follow-up, there was no difference in the incidence of the composite of death, MI, or stroke after FFR-guided PCI with current-generation drug-eluting stents compared with CABG. There was a higher incidence of MI after PCI compared with CABG, with no difference in death or stroke. These results provide contemporary data to allow improved shared decision-making between physicians and patients with 3-vessel coronary artery disease.REGISTRATION: URL: https://www.CLINICALTRIALS: gov; Unique identifier: NCT02100722.PMID:37602376 | DOI:10.1161/CIRCULATIONAHA.123.065770

Treatment of significant carotid artery stenosis in 1824 patients.
Conclusion: In our experience, CAS offered a valid alternative for both symptomatic and asymptomatic patients who were poor candidates for CEA, with results that compared favourably to those of CEA both at 30-day and at long-term. Patients who couldn't be operated on neither with CAS nor with CEA had a lower risk of MI at 30-day but a higher risk of stroke during the first year, especially if they had previously experienced neurological symptoms. Use of a novel embolic filter in carotid artery stenting: 30 ‐Day results from the EMBOLDEN Clinical Study
ConclusionIn patients at high risk for CEA undergoing CAS, the GEF not only showed high rates of successful deployment but also met the primary endpoint of low death, stroke, and MI rates, thus demonstrating safety and effectiveness.

Carotid Revascularization with and without the Use of an Embolic Protection Device: A Single-Center Experience from Pakistan
Conclusion: Based on our single-center experience and findings of a relatively small sample size, carotid revascularization with stenting and angioplasty without EPD in experienced hands was found to be safe and efficacious. In addition, it proves cost-effective for patients by limiting the use of unnecessary disposables. These results are comparable to those reported in major trials and are well within the complication thresholds suggested in current guidelines. These results also show promise and illustrate the need for a larger, randomized controlled trial in order to thoroughly address this aspect of carotid revascular...

Abstract 312: Anatomic Runoff Score Predicts Cardiovascular Outcomes in Patients with Lower Extremity Peripheral Artery Disease Session Title: Poster Session III
Conclusions: After adjustment for clinical factors, the LE-PAD anatomic runoff score, age and indication for procedure were the most significant predictors of future cardiovascular morbidity and mortality in a broadly-representative patient population undergoing revascularization for symptomatic PAD. The use of a clinically useful anatomic scoring system, if validated, may assist clinicians in risk-stratifying patients during the course of clinical decision-making.

Long-Term Outcome of PCI Versus CABG in Insulin and Non–Insulin-Treated Diabetic Patients Results From the FREEDOM Trial
BackgroundThe prospective, randomized FREEDOM (Comparison of Two Treatments for Multivessel Coronary Artery Disease in Individuals With Diabetes) trial found coronary artery bypass graft surgery (CABG) was associated with better clinical outcomes than percutaneous coronary intervention (PCI) in patients with diabetes and multivessel disease, managed with or without insulin.ObjectivesIn this subgroup analysis of the FREEDOM trial, we examined the association of long-term clinical outcomes after revascularization in patients with insulin-treated diabetes mellitus (ITDM) compared with patients not treated with insulin.Method...
