Routine Glycoprotein IIb/IIIa Inhibitor Therapy in ST-Segment Elevation Myocardial Infarction: A Meta-Analysis
ConclusionsRoutine GPI administration in STEMI resulted in mortality reduction, driven by reductions in recurrent ischemic events – however predominantly in trials pre-prasugrel/ticagrelor. Trials in contemporary STEMI management are needed to confirm these findings.

GFR-specific cut-offs can refine the prognostic value of circulating cardiac biomarkers in advanced chronic kidney disease
ConclusionsAmong asymptomatic patients with advanced CKD, optimal cut-offs for hs-cTnT and NT-proBNP differed by GFR level and out-performed standard cut-offs for discrimination of cardiovascular risk.

GFR-Specific versus GFR-Agnostic Cutoffs for Parathyroid Hormone and Fibroblast Growth Factor-23 in Advanced Chronic Kidney Disease
Conclusion: GFR-specific risk-based cutoffs for PTH1 –84 and FGF-23 may facilitate more meaningful risk stratification in advanced CKD than current GFR-agnostic reference ranges derived from healthy adults. This may be most applicable in those with severely reduced GFR.Am J Nephrol

Genetics of hyperhomocysteinemia and 1-carbon metabolism: implications for retinal structure and eye functions.
Abstract Homocysteine (Hcy); a sulfur-containing non-proteinogenic amino acid is generated as a metabolic intermediate. Hcy constitutes an important part of the "1-carbon metabolism" during methionine turnover. Elevated levels of Hcy known as hyperhomocysteinemia (HHcy) results from vitamin B deficiency, lack of exercise, smoking, excessive alcohol intake, high fat and methionine rich diet, and the underlying genetic defects. These factors directly affect the "1-carbon metabolism (methionine-Hcy-folate)" of a given cell. In fact, the Hcy levels are determined primarily by dietary intake, vitamin status, and the ge...

Glomerular Filtration Rate-Specific Cutoffs Can Refine the Prognostic Value of Circulating Cardiac Biomarkers in Advanced Chronic Kidney Disease
Nou...

Peritonitis affects the relationship between low-density lipoprotein cholesterol and cardiovascular events in peritoneal dialysis patients
ConclusionsPD patients with lower baseline LDL-C had a higher risk of peritonitis. The effect of LDL-C on CV events and mortality was different by the presence of peritonitis events.

Podcast: Pharmacotherapy for hypertension in adults 60 years or older
One of the earliest Cochrane Reviews on high blood pressure was first published in 1998, investigating treatments for the elderly. It was substantively updated for a second time in June 2019 and we asked the lead author, Vijaya Musini, from the University of British Columbia in Vancouver Canada to tell us about the evidence.High blood pressure or hypertension, which is defined as systolic blood pressure of 140 mm of mercury or more, or diastolic blood pressure of 90 mm of mercury or more, increases with age, particularly in those over 60. It raises the risk of heart attack and stroke, with systolic hypertension, which is m...

Association of predicted lean body mass and fat mass with cardiovascular events in patients with type 2 diabetes mellitus.
Authors: Xing Z, Tang L, Chen J, Pei J, Chen P, Fang Z, Zhou S, Hu X Abstract BACKGROUND: Previous studies have found that predicted fat mass and lean body mass may act differently on adverse events. However, the cardiovascular prognostic value of lean body mass and fat mass in patients with type 2 diabetes mellitus (T2DM) has not yet been investigated. We sought to investigate the relation between predicted lean body mass or fat mass and the risk of cardiovascular disease in patients with T2DM. METHODS: We conducted a post hoc analysis of data from the Action to Control Cardiovascular Risk in Diabetes (ACCORD)...

Dual antithrombotic therapy with DOACs after ACS or PCI in Atrial Fibrillation: A meta-analysis of Randomized Controlled Trials
ConclusionsIn patients with AF and recent ACS or PCI, the use of dual antithrombotic therapy with DOACs was associated with less major bleeding, and major bleeding or clinically relevant non-major bleeding, compared to triple therapy. The use of dual therapy also shows non-significantly higher composite of death/ischemic events but no difference in mortality.

Clinical impact of valvular heart disease in elderly patients admitted for acute coronary syndrome: insights from the Elderly-ACS 2 Study
ConclusionsIn a contemporary cohort of elderly patients admitted for ACS, VHD was found in one out of five subjects and had an independent, consistent impact on prognosis.

The Cardiovascular Effects of SGLT2 Inhibitors
Publication date: Available online 24 September 2019Source: Canadian Journal of DiabetesAuthor(s): Vincent C. WooAbstractIndividuals with type 2 diabetes mellitus can benefit from pharmacotherapies that lower their risk for cardiovascular disease. This review describes the salient findings from sodium-glucose cotransporter-2 (SGLT2) inhibitor cardiovascular outcome trials that serendipitously revealed the cardiorenal benefits of SGLT2 inhibitors in individuals with type 2 diabetes mellitus who either have established cardiovascular disease or multiple cardiovascular risk factors. It also summarizes the findings from other ...
