ConclusionsHybrid aortic arch and FET repair with the Thoraflex hybrid graft appears to be associated with good clinical outcomes, despite being early in the learning curve with this graft. Further investigation with this device is warranted to establish its role within the variations of hybrid arch repair.

A Rare Cause of Stroke FourWeeks After Ascending Aortic Aneurysm Repair
A 71-YEAR-OLD, 97 kg, 178 cm man with a history of a slowly expanding 5.4 cm ascending aortic aneurysm, bicuspid aortic valve disease without stenosis, hypertension, hyperlipidemia, type II diabetes mellitus, and hemodynamically insignificant coronary artery disease underwent ascending aortic replacement using a woven synthetic graft. The patient's postoperative course was complicated by persistent sternal drainage that required exploration, evacuation of pericardial fluid, and antibiotic treatment, but he otherwise made an uneventful recovery.

Early Outcomes After Branched and Fenestrated Endovascular Aortic Repair in Octogenarians.
CONCLUSIONS: Complex endovascular repair in octogenarians has higher early all cause mortality compared with non-octogenarians. Rupture and higher ASA class of ≥4 are independent predictors for early mortality. Age ≥80 years was found to be an independent predictor for higher early all cause mortality. Urgent Endovascular Treatment for Non-traumatic Descending Thoracic Aortic Rupture

Abstract Diagnosis of abdominal aortic aneurysm (AAA) at preoperative stage is increasingly frequent. It carries both a local risk of rupture and an increased global cardiovascular risk. Patients with AAA have indeed a 20 times higher risk of dying from myocardial infarction or stroke than from a ruptured aneurysm. Total arch replacement versus debranching thoracic endovascular aortic repair for aortic arch aneurysm: what indicates a high-risk patient for arch repair in octogenarians?
ConclusionsTAR in octogenarians with COPD and/or malignancy showed higher mortality rates; d-TEVAR is more appropriate in these situations. The prevention of perioperative stroke, which is related with poor prognosis in both the groups, is critical.
