Skin Barrier Dysfunction In Patients With Peanut Allergy Without Eczema
Atopic dermatitis (AD+) with peanut allergy (PA+) is associated with increased transepidermal water loss (TEWL), low urocanic acid (UCA, a filaggrin breakdown product), and reduced ratio of EOS-ceramides to NS-ceramides. Here we explored whether PA without AD (AD-PA+) also had skin barrier abnormalities.

POEM correlates with EASI and other measures better than PO-SCORAD: A validation study.
CONCLUSION: PO-SCORAD and POEM showed good construct and cross-cultural validity, reliability, and responsiveness in adults with AD, and were feasible for use in clinical trials and practice. A real-world study of the longitudinal course of adult atopic dermatitis severity in clinical practice.
CONCLUSION: AD lesional severity has a heterogeneous longitudinal course. Many patients had fluctuating lesional severity scores over time. A minority of patients had persistently moderate or severe lesions over time. Most patients with moderate-severe disease at baseline were unable to achieve persistent lesional clearance. The Infectious Complications of Atopic Dermatitis.
CONCLUSION: Infectious complications are a co-morbidity of AD. Although not common, systemic bacterial infections and eczema herpeticum can be life-threatening. Preventive therapy of infections in AD emphasizes skin barrier improvement and anti- inflammatory therapy. The use of antibiotics in AD exacerbation requires further studies. Efficacy, Safety, and Immunomodulatory Effect of the Intramuscular Administration of Autologous Total Immunoglobulin G for Atopic Dermatitis: A Randomized Clinical Trial.
CONCLUSIONS: The intramuscular administration of autologous total IgG provided clinical improvements and a systemic immunomodulatory effect in adolescent and adult patients with moderate-to-severe AD without significant side effects. Evaluation of Olfactory Function in Children With Vernal Keratoconjunctivitis.

Leveraging shared decision making to discuss nonessential medical testing and prevent peanut allergy overdiagnosis during infancy
We read with interest the recent report by Keet et  al1 describing the use of Ara h 2–specific IgE (sIgE) as a peanut allergy screening tool in infants with moderate to severe eczema, compared with peanut skin prick test and whole peanut sIgE.1 The authors report a sensitivity of 94% and near-perfect specificity of 98% at an “optimal” 0.1 kU/ L cutoff value for Ara h 2-sIgE in this sample. At 0.35 kU/L, these values were 89% (sensitivity) and 99% (specificity).1 The trial was conducted primarily at 1 center and enriched for infants with moderate to severe eczema.
