Current Clinical Trials in Pemphigus and Pemphigoid
Conclusions The clinical trials discussed here, which include several trials investigating novel therapeutic targets, demonstrate that translational research in pemphigus and pemphigoid is a fast-growing field. We thus expect that several novel treatments will be shortly available for the treatment of pemphigus and pemphigoid patients. Given the high, and thus far unmet, medical need in this field (110), this is highly encouraging and will hopefully improve the quality of life of the affected patients. Conclusions Though this is not a comprehensive summary of malignancies in PIDDs, or even lymphoproliferative disease in this area, this review summarizes the Medline-indexed published reports of B and T lymphomas in patients with PIDDs. Discussion Making this diagnosis is challenging, as brain imaging often shows non specific changes and blood results are usually normal. CSF may reveal raised proteins and WBCs. A high index of suspicion is required to arrange a brain biopsy as this confirms the diagnosis.

Neuromyelitis optica in hiv: a case report and literature review

A 49 year old lady of African descent with known HIV diagnosed in 2004 – compliant with her antiretroviral medication, Atripla – was admitted with a 3 week history of thoracic back pain, spreading paraesthesia and weakness involving both legs, and constipation. Examination revealed a spastic paraparesis, a sensory level at T3/4 on the right and reduced vibration sense to the iliac crests bilaterally. Hypersensitivity pneumonitis following alemtuzumab treatment for multiple sclerosis
We present two patients diagnosed with hypersensitivity pneumonitis following alemtuzumab therapy for multiple sclerosis. Both patients presented with a sub-acute pulmonary illness characterized by cough and breathlessness with interstitial pulmonary infiltrates subsequently demonstrated on chest x-ray and computed tomography of the thorax. Both patients responded well to oral prednisolone and in the case of one patient for whom serum avian precipitins had been strongly positive, removal of a pet parrot. A 39 year old lady presented with an altered mental state and unsteadiness of gait. She had a history of juvenile idiopathic arthritis, autoimmune thyroiditis and alopecia, protein S deficiency with vena cava thrombosis, previous excision of ovarian mass and vasculitic rash.
