Reduction in estimated glomerular filtration rate (eGFR) results in an increased risk of spontaneous hemorrhagic transformation in patients with large-artery atherosclerosis stroke.
In conclusion, reduced eGFR was significantly associated with increased risk of spontaneous HT in all ischemic stroke patients, and in large artery atherosclerosis, not in cardio-embolism stroke. Reduced eGFR did not increase the risk of symptomatic HT. PMID: 26503023 [PubMed - as supplied by publisher]

Addition of sodium criterion to SOAR stroke score
ConclusionThe SOAR‐Na did not perform considerably better than the SOAR stroke score. However, the performance of SOAR‐Na in those with CKD and dysglycaemias requires further investigation.

Dabigatran Reversal with Idarucizumab Preceding Thrombolysis in an Octogenarian Patient with Chronic Kidney Disease and Acute Stroke: a Case Report.
CONCLUSION: Our case showed the effectiveness and safety of giving Idarucizumab followed by thrombolysis in Dabigatran-treated atrial fibrillation with ischemic stroke. Based on this case, the procedure can be performed in an elderly population with chronic kidney disease when administered close to the limit of threshold for thrombolysis. Serpiginous cranial arterial calcification in uremia
A 65-year-old man with chronic kidney disease (CKD) on hemodialysis presented with a subcortical stroke. Unenhanced CT scan of the brain revealed widespread arterial calcification, notably involving the carotid siphons (figure 1) and external carotid artery branches (figure 2, A and B). Vascular calcification in CKD closely resembles ossification, and may involve the intima (as in atherosclerosis) or more commonly the media.1 Both patterns of calcification are associated with increased cardiovascular mortality, though no specific treatment has been shown to modify this risk.2 This example is particularly striking for the s...

Atypical presentation of fistula dysfunction due to brachial arterial embolization mimicking stroke
In this report, failure to recognize the upper limb ischemia would have led to delayed treatment, potentially resulting in the fistula's complete thrombosis and further limb ischemia. We highlight the importance of a diagnosis method like Doppler ultrasound, which allows for rapid evaluation at the patient's bedside.PMID:35000475 | DOI:10.1177/11297298211067686

A Case of Transient Global Amnesia: A Review and How It May Shed Further Insight into the Neurobiology of Delusions
Conclusion In closing, our patient’s episode of TGA combined with her emotional and perceptual response lends credence to the proposal of a “fear/paranoia” circuit in the genesis of paranoid delusions—a circuit incorporating amygdala, frontal, and parietal cortices. Here, neutral or irrelevant stimuli, thoughts, and percepts come to engender fear and anxiety, while dysfunction in frontoparietal circuitry engenders inappropriate social predictions and maladaptive inferences about the intentions of others.[54] Hippocampus relays information about contextual information based on past experiences and the current situat...

Poor Long-term Outcomes after Carotid Endarterectomy: A Retrospective Analysis of Two Portuguese Centers.
CONCLUSIONS: Patients continue to have high rates of AMI, MACE and death after CEA. Prior CAD is a risk factor for future AMI, whereas CKD is a significant predictor of MACE and death. Aggressive best medical treatment (BMT) and risk factors modification should be advised in patients with systemic atherosclerosis. PMID: 31251084 [PubMed - as supplied by publisher]

Intracranial Artery Calcification and Its Clinical Significance.
Authors: Wu XH, Chen XY, Wang LJ, Wong KS Abstract Intracranial arterial calcification (IAC) is an easily identifiable entity on plain head computed tomography scans. Recent studies have found high prevalence rates for IAC worldwide, and this may be associated with ischemic stroke and cognitive decline. Aging, traditional cardiovascular risk factors, and chronic kidney disease have been found to be associated with IAC. The severity of IAC can be assessed using different visual grading scales or various quantitative methods (by measuring volume or intensity). An objective method for assessing IAC using consistent cr...

Contrast induced encephalopathy mimicking reperfusion injury (P3.285)
Conclusions:Diagnostic or therapeutic angiography is frequently performed for acute ischemic or hemorrhagic stroke. Clinicians should be aware of contrast induced encephalopathy and consider it in cases of neurologic deterioration after angiography in order to initiate appropriate treatment and avoid unnecessary invasive tests.Disclosure: Dr. Bakradze has nothing to disclose. Severe Symptoms, but a Truly Treatable Disease
BY NOURA MAHDI; DARRON LEWIS; JEREMY OSBORNE; & AHMED RAZIUDDIN, MDA 73-year-old man was brought to the emergency department from his nursing home for rectal bleeding and anemia. The patient mentioned he had had episodes of bright red rectal bleeding and constipation for a few months. A colonoscopy had been done prior to the visit, which revealed a large intestine tumor and biopsy confirming adenocarcinoma. He was awaiting an appointment with his surgeon. The patient reported bloody rectal leakage, and a CBC done at the nursing home showed a hemoglobin level of 7.2. He also complained of dyspnea but denied any other ...

Benefits and Harms of Oral Anticoagulant Therapy in Chronic Kidney Disease: A Systematic Review and Meta-analysis.
Conclusion: In early-stage CKD, NOACs had a benefit-risk profile superior to that of VKAs. For advanced CKD or ESKD, there was insufficient evidence to establish benefits or harms of VKAs or NOACs. Cardiovascular highlights from non-cardiology journals
This study included randomised trials of drugs to lower blood pressure that were compared with placebo or other drugs with at least 1000 patient years of follow-up in each treatment arm. The primary outcome was major CV events (the composite of stroke, myocardial infarction, heart failure or CV death), with secondary outcomes of each element of the composite outcome as well as all-cause mortality. These 26 trials included 152 290 patients, of which 30 295 had impaired renal function (estimated glomerular filtration rate (eGFR) of <60 mL/min/1.73 m2). Meta-analysis according to baseline kidney fu...
