Clinical effectiveness of the systematic use of the GRACE scoring system (in addition to clinical assessment) for ischaemic outcomes and bleeding complications in the management of NSTEMI compared with clinical assessment alone: a prospective study
Abstract We assessed the interest of systematically using the GRACE scoring system (in addition to clinical assessment) for in- hospital outcomes and bleeding complications in the management of NSTEMI compared with clinical assessments alone. Multicentre, randomized study that included 572 consecutive NSTEMI patients, randomized 1:1, into group A: clinical stratification alone and group B: clinical+ GRACE score stratification. Main outcome measures: in-hospital outcomes and bleeding complications. There was no significant difference between the two groups for baseline data or for in-hospital MACE. In multivaria...

Impact of Arterial Access Site on Outcomes After Primary Percutaneous Coronary Intervention: Prespecified Subgroup Analysis From the EUROMAX Trial Coronary Interventions
Conclusions— In this prespecified analysis from EUROMAX, radial access was preferred in lower risk patients and did not improve clinical outcomes. Bivalirudin was associated with less bleeding irrespective of access site. Clinical Trial Registration— URL: http://www.clinicaltrials.gov. Unique identifier: NCT01087723.

Pediatric giant cell glioblastoma mimicking hemorrhage secondary to ischemic stroke
An eight-year-old girl presented with headaches, nausea, and diminished consciousness. Initial computed tomography (CT) showed an intracerebral hemorrhage with mass effect (Fig. 1). A four-vessel catheter angiogram ruled out vascular malformations, aneurysms or vasculitis. On magnetic resonance imaging (MRI), the hemorrhage had both an acute and sub-acute bleeding component (Fig. 2A and 2B). Anteriorly of the hemorrhage, a region with weak contrast enhancement was noted (Fig. 2C and 2D). This was interpreted as reactive contrast enhancement.

Radial versus Femoral Access for Elderly Patients with Acute Coronary Syndrome undergoing Coronary Angiography and Intervention: Insights from the RIVAL Trial
Conclusions Consistent with the overall RIVAL trial population, elderly patients undergoing cardiac catheterization have lower rates of major bleeding or access site complications and higher rates of access site crossover with radial access compared to femoral access.

Rate of Contrast Extravasation on Computed Tomographic Angiography Predicts Hematoma Expansion and Mortality in Primary Intracerebral Hemorrhage Clinical Sciences
Conclusions— Contrast extravasation rate, or spot sign growth, further refines the ability to predict hematoma expansion and mortality. Our results support the hypothesis that the spot sign directly measures active bleeding in acute intracerebral hemorrhage.

Angiographic factors influencing the success of endovascular treatment of arteriovenous malformations involving the corpus callosum
Conclusions AVMs of the corpus callosum seems to be difficult to treat with endovascular therapy alone. The goal of embolization should be prevention of (re) bleeding and a decrease in nidus size. Our experience regarding this sub-pathology suggests that a combination of endovascular therapy and radiotherapy may be the best option.

Clopidogrel Versus Aspirin as an Antiplatelet Monotherapy After 12-Month Dual-Antiplatelet Therapy in the Era of Drug-Eluting Stents Pharmacology
Conclusions— After 12-month DAPT, clopidogrel monotherapy, when compared with aspirin monotherapy, might be associated with a reduced risk of recurrent ischemic events in patients receiving DES.

Arrive: A retrospective registry of Indian patients with venous thromboembolism
Conclusion: Acute DVT alone was responsible for the substantial burden of VTE in Indian patients. Bleeding was not the limiting factor for anticoagulant treatment in most patients.

Intra-Arterial Stem Cell Therapy for a Persistent Vegetative State Patient: A DTI Analysis of Recovery (P6.066)
Conclusion Assessing minor neurological improvements in this patient remains challenging but advances in neuroimaging as an evaluating tool helps to detect those changes. Ticagrelor and aspirin for the prevention of cardiovascular events after coronary artery bypass graft surgery
Conclusions Ticagrelor added to aspirin after CABG reduced the proportion of patients with graft occlusion, and was a significant univariate and multivariable predictor of graft occlusion. These results are hypothesis-generating and should be confirmed in larger studies. Trial registration number NCT01373411: Results.

Is emergency and salvage coronary artery bypass grafting justified? The Nordic Emergency/Salvage coronary artery bypass grafting study ADULT CARDIAC
CONCLUSIONS Early mortality in patients undergoing emergent and salvage CABG is substantial, especially in salvage patients. Long-term survival is acceptable in both emergent and salvage patients. Life-saving emergency and salvage CABG is justified in most patients but salvage patients have dismal prognosis if cardiac massage is needed during sternotomy.

Acute kidney injury in acute coronary syndromes – An important multifactorial consequence
Conclusions AKI is an important finding in ACS, with a significant impact on hard clinical endpoints such as in-hospital and one-year mortality. It is associated with easily identifiable clinical factors and an invasive strategy does not increase its incidence.
