Fight Aging! Newsletter, February 27th 2023
This study tested the hypothesis that ischemic vascular repair in aging by Ang-(1-7) involves attenuation of myelopoietic potential in the bone marrow and decreased mobilization of inflammatory cells. Young or Old male mice of age 3-4 and 22-24 months, respectively, received Ang-(1-7) for four weeks. Myelopoiesis was evaluated in the bone marrow (BM) cells by carrying out the colony forming unit (CFU-GM) assay followed by flow cytometry of monocyte-macrophages. Expression of pro-myelopoietic factors and alarmins in the hematopoietic progenitor-enriched BM cells was evaluated. Hindlimb ischemia (HLI) was induced by ...

Post-transplantation lymphoproliferative disorders in renal vs. simultaneous renal-pancreas allograft recipients: A survey and analysis of data from the literature.
In this study, we sought to determine the clinical, histopathological and various other features of PTLD in recipients of pancreas-renal allografts and to compare their data with renal-only transplant patients, based on the current available literature. We conducted a comprehensive search for the available data using the Pubmed and Google scholar search engines for reports of lymphoproliferative disorders after renal and simultaneous pancreas-renal (SPR) transplantations. A total of 229 recipients of renal and pancreas-renal allografts were included in the analysis. Localizations for SPR recipients were significantly highe...

Thyroid hormones for acute kidney injury.
CONCLUSIONS: We found a paucity of large, high quality studies to inform analysis of thyroid hormone interventions for the treatment of people with AKI. Current evidence suggested that thyroid hormone therapy may be associated with worse outcomes for patients with established AKI; therefore, its use for these patients should be avoided. The role of thyroid hormone therapy in preventing AKI has not been adequately investigated and may be considered in future clinical studies. PMID: 23440810 [PubMed - in process]

Small intestinal involvement by lymphoproliferative disorders post-renal transplantation: A report from the post-transplant lymphoproliferative disorder international survey.
In this study, data on post-renal transplant lymphoproliferative disorders (PTLD) collected from the existing literature were pooled and analyzed to compare the characteristics, predictors and prognosis of small intestinal PTLDs. We performed a comprehensive search for the available data by Pubmed and Google scholar search engines for reports on this subject. Data from 18 previously published studies, comprising 120 renal allograft recipients, were included in the analysis. Renal transplant recipients with intestinal PTLD were significantly less likely to have Hogkin's and Hogkin's-like lesions (P = 0.044) and to be younge...

Post-transplantation lymphoproliferative disorders (PTLD) localized in the central nervous system: report from an international survey on PTLD.
This study showed that PTLD patients who had CNS presentation have quite a comparable outcome compared with those with other areas of localization. However, further prospective studies are needed for reaffirming our findings. PMID: 23538344 [PubMed - in process]

Characteristics and prognosis of lymphoproliferative disorders post-renal transplantation in living versus deceased donor allograft recipients.
In this study, we compared the features and prognosis of post-transplantation lymphoproliferative disorders (PTLD) occurring in living donor recipients with those of deceased donor kidney transplant patients. A comprehensive search was performed for finding studies reporting data of PTLD in living and deceased donor renal recipients in the Pubmed and Google scholar search engines. Finally, international data from 14 different studies were included in the analysis. Overall, 122 renal recipients with PTLD were entered into this analysis. Chi square test showed that renal recipients from living donors significantly less frequ...

Review article: nucleos(t)ide analogues in patients with chronic hepatitis B virus infection and chronic kidney disease
ConclusionsChronic hepatitis B patients with chronic kidney disease receiving nucleoside analogues should be followed carefully for treatment efficacy and renal safety. Despite the absence of strong data, entecavir and telbivudine seem to be the preferred options for nucleoside analogue‐naive CHB patients with chronic kidney disease, depending on viraemia and severity of renal dysfunction. More studies are certainly needed in this setting.

Critical Care Compendium update
LITFL’s Critical Care Compendium is a comprehensive collection of pages concisely covering the core topics and controversies of critical care. Currently there are almost 1,500 entries with more in the works… Some pages are more developed than others, and all the pages are being constantly revised and improved. Links to new references and online resources are added daily, with an emphasis on those that are free and open access (FOAM!). These pages originated from the FCICM exam study notes created by Dr Jeremy Fernando in 2011, and have been updated, modified and added to since. As such will be particularly us...

Ultra-early onset post-transplantation Lymphoproliferative disease.
This study, however, tries to research PTLD characteristics including histopathological and clinical features, predictors and prognosis of the disease when occurring within the first month post-transplantation. We conducted a comprehensive search for the available data using the Pubmed and Google scholar search engines for reports indicating presentation time in PTLD patients. Data from 25 previously published studies were included in the analysis. Finally, we found 355 recipients of organs presenting with "ultra-early onset PTLD." Transplant recipients with ultra-early onset PTLD were significantly more likely to have kid...

Localization of post-transplant lymphoproliferative disorders to the stomach might be associated with favorable outcome: A systematic review.
In this study, we aimed to accumulate existing data in the current literature to reveal the clinical, histopathological and prognostic specificities associated with gastric PTLDs and to find the best treatment strategies in this patient population. A comprehensive search was conducted for the available data in the current literature using Pubmed and Google scholar search engines for reports on gastric PTLD in renal transplant recipients. Data of different studies were standardized and entered into a database and analyzed. No statistically significant difference was found between gastric and non-gastric PTLD. Gastric PTLD w...

Aldosterone antagonists for preventing the progression of chronic kidney disease.
CONCLUSIONS: Aldosterone antagonists reduced proteinuria and blood pressure in adults who had mild to moderate CKD and were treated with ACEi or ARB (or both), but increase hyperkalaemia and gynaecomastia. Whether adding aldosterone antagonists to ACEi or ARB (or both) reduced the risk of major cardiovascular events or ESKD in this population is unknown. Steroid avoidance or withdrawal for pancreas and pancreas with kidney transplant recipients.
CONCLUSIONS: There is currently insufficient evidence for the benefits and harms of steroid withdrawal in pancreas transplantation in the three RCTs (144 patients) identified. The results showed uncertain results for short-term risk of rejection, mortality, or graft survival in steroid-sparing strategies in a very small number of patients over a short period of follow-up. Overall the data was sparse, so no firm conclusions are possible. Moreover, the 13 observational studies findings generally concur with the evidence found in the RCTs. Kidney swap saves two lives, eternally links two families
When Dillon McCarty was 13, he received a very special gift from his stepfather Nicholas Gula—a new kidney. But Gula, 34, didn’t donate his kidney to Dillon. Instead, he gave it to a stranger, a 29-year-old man also in need of a kidney transplant in Atlanta. And in return, the man’s wife gave her kidney to Dillon. The carefully choreographed “operation” involved two selfless donors, two flights, two states, three hospitals and four surgeries—more than one thousand miles apart—inexplicably tying two families. “It was a dream,” says Gula. “I was saving two lives. Words can’t even express how I felt....

Interventions for dialysis patients with hepatitis C virus (HCV) infection.
CONCLUSIONS: Our review demonstrated that in CKD patients on haemodialysis with HCV infection treatment with standard interferon brings about an end of treatment but not a sustained virological response and is relatively well tolerated. PEG interferon is more effective than standard interferon for end of treatment response but not for sustained response; both were equally tolerated. Increasing doses of PEG interferon did not improve responses but high and low doses are equally tolerated. Addition of ribavirin results in more treatment discontinuation. Increasing Transplant Organ Supply Through Uncontrolled Donation After Cardiac Death
This study would suggest that a publicly accepted approach to uDCD would be to require consent prior to organ preservation, even if this is not legally or ethically required. Should uDCD Protocols Be Pursued In The US? Uncontrolled donation after cardiac death protocols provide an opportunity to expand the organ donor pool, thereby decreasing waiting list time and improving outcomes for potential organ recipients. Given that the waiting list for organ donation has not significantly changed in the past several years, and thousands of people continue to die or have a poor quality of life while awaiting organs, efforts should...
