ABIM and MOC status - wtf?
So when I originally took the IM certification exam a while back, my understanding was that you could either take the test every 10 years OR participate in MOC. My choice was to take the test every 10 years (actually, my choice was to let the IM cert lapse in 10 years because I held the rheumatology cert, but I digress). Thinking about doing a Endocrine vs Rheumatology fellowship vs just starting PCP practice. Soon to be PGY2 IM Resident.
So I'm an IM PGY2 soon to be PGY3 resident... I am thinking about doing an Endocrine vs Rheumatology fellowship. I don't have a ton of research but I have good Step 1 and 2 Scores as a DO resident. I like the Endocrine system and Rheum is cool as well, but pondering whether its a good idea to delay making decent money for another 2 years before I can start earning. XRT for spine arthritis, or just extremities?
We've discussed many times here the merits of low dose RT for arthritis, but it seems to always be extremity. I guess my in-house program has taken off too well, because I got a referral for refractory spine arthritis pain, in a patient requiring interventions like nerve ablations. I got a job offer, need help from more seasoned rheumies.
So I happen to be on vacation, coastal MA in a town where my spouse is from and doesn't mind moving back to...blah blah blah. So naturally I looked into the local hospital system and noticed they were looking to hire 3 new rheumatologists for 2024. Rheumatology and long term steroids
I did a month of rheumatology in residency and I don’t remember this being a thing. I have noticed that two local rheumatologists in different cities near me that right after they diagnose someone with autoimmune disease, even things without a firm diagnosis (like undifferentiated connective tissue issues) They will start plaquenil or methotrexate, which is fine, but they also put patients on oral steroids for 6-8 weeks!! Questions about Endocrinology and Rheumatology Work-Life, Job Outlook, etc.?

TIPS for ILESI - difficult arthritic back
Had a patient that had extremely arthritic spine, difficult to see due to OA/osteophytes Is there some technique to orient yourself in the spine with this type of back (sorry, no pictures saved, but in future if I see them, i'll post it). Advanced Hip AVN options?
Have a patient with a bad hip, not a surgical candidate. MRI Impression 1. Severe left hip arthritis with moderate flattening of the femoral head contour suggestive of advanced AVN 2. Allopurinol Risk Assessment
Question for other PCP’s, Rheumatologists, or other Allopurinol prescribers. Lexicomp recommends screening not only persons of certain Asian ancestry, but also all African Americans for the HLA-B*5801 allele prior to starting Allopurinol given the higher prevalence of the allele in these populations and the association with severe Allopurinol hypersensitivity reactions.
