MSTP student interested in Neurology, doubts due to salary
I have been accepted into a few MD/PhD programs and was always very focused on pursuing a primarily research career. I am very interested in working with patients with rare genetic illnesses and optimizing therapeutics for these patients. One aspect that really motivates me is the lack of physicians with similar interests, since most will focus on the big neuro diseases/injuries, such as TBI, strokes, parkinson's, alzheimers. PR-interval - What happens when there's no Q-wave?
Hey, So for Wolff-Parkinson-White syndrome I stumbled upon this image in FA: PR-interval is from P-wave to Q-wave. However, there is no Q-wave in that image (negative deflection prior to R-wave). How can PR-interval then be marked as it is in the image? (Source: Student Doctor Network)

Tremor in Parkinson
So the way Kaplan explains Parkinson dz, is that this is the dz in direct basal ganglion pathway. Lack of DA --> suppressed direct pathway --> suppressed motor cortex --> rigidity Which all makes sense. However, how do you explain tremor? Isn't this counter intuitive if Dopamine plays a role in facilitying direct pathway which facilitates movement, such as tremor? According to FA2CK, Dopamine Agonists used in the treatment of Parkinson Disease, such as Ropinirole, Pramipexole, can cause compulsive gambling as a side effect. Antidepressant for parkinsons patient?
So I got a patient who has had long standing depression. She has been on 60 mg of Cymbalta and was weaning down to 40 because it stopped working. She wants med management and therapy, I like therapy and will be seeing her every other week for therapy- she can only come every other week for therapy due to transportation issues. The first step was easy, titrate down to 20 mg of Cymbalta. NO antidepressant works with her parkinsons meds because they are all a risk for serotonin syndrome.... Let family history determine specialty?
I am about to start 3rd year rotations, and have begun to put a lot of thought into choosing a specialty. Although my Step score probably narrowed down what I'm competitive for, I'm stuck between the age old choice of medicine vs surgery.
