Endothelial Dysfunction?
If someone has suspected Endothelial Dysfunction due to a hyperentension emergency that wasn''t treated properly, what tests would you give to investigate, and what meds would you prescribe? So I'm trying to decide on stanford vs UCSF vs baylor vs cinci I'm looking to go into subspecialty peds and work in academics stanford- feel like I get best research experience with great subspecialty but mediocre gen peds training, hate living in suburb UCSF- great research and training but living in SF seems like a huge headache baylor- have ties to area, seems like good all around training but not quite as prestigious as other programs or as good research Cinci- program great all around... more rank list help (Source: Student Doctor Network)

Case: Stroke-like symptoms with headache
Case: 70+ yo F w/ hx of CVA 7yrs prior w/ residual mild expressive aphasia presents w/ severe expressive and receptive aphasia and severe headache. Onset 1hr prior to arrival, and the onset was actually witnessed by family member. The family member at bedside states she went from calmly eating her breakfast to placing her head in her hands in obvious, severe discomfort with an inability to communicate. I have a few questions about Step 2 CS
1) Is it absolutely necessary to do Heart, Lung and Abdominal examination for every case? Like even for a case of migraine or tension headaches? I ask because I am seeing people (on older posts of forums) who passed and said they did CVS, Respi and Abd examination for every case regardless. 2) Is it necessary to ask Ob-Gyn history for every female patient regardless of their presentation? 3) Is it necessary to make small talk...
