Valid license/immunizer but no npi
Started working part-time at this small independent. My license is active and I am a certified immunizer but my npi is not active. I remember at cvs I was giving shots without an npi and was never asked to register for one. Will this be an issue ? I know sp/pic definitely needs npi for billing and things like that but didn’t realize the staff Rph also did too ? Ny state…Please advise. Thank you . (Source: Student Doctor Network)

Shenanigans ahead of Surprise Billing legislation
This was sent to 50+ providers in North Carolina from BCBS (and verified). Pay attention to what the payors will do even before SBL takes effect. They can basically control QPA, so they can control the “fair market” rate, and will demand practices reduce their rates to stay in network or face arbitration to be paid their “fair market” rates set by QPA. Please take time to contact your local societies and legislators so they may apply pressure to delay the rollout of SBL and... Read more (Source: Student Doctor Network)

ACFAS coding seminar
I've seen much discussion of coding lately, but haven't seen much as far as seminars goes. I'm going to be going to ACFAS in Austin this next spring for the first time, and as a graduating resident, and am interested in this coding for noobs seminar they are running. Can anyone speak to whether it is likely worth an extra $125 and flying in early for, or if perhaps there is a better/cheaper seminar that would be more beneficial? I plan to open my own place, so billing and coding will be... Read more (Source: Student Doctor Network)

Should I take this job?
Graduating C&A fellowship. Salary: Non-guaranteed, I get 70% of what I bring in. Typically, which should be about $310K per year based on other providers. Practice only accepts high level private insurance. There is dedicated billing staff who collect payments. Insurance turn around is usually 2 weeks, I've been told, but can obviously vary with deductibles and such. Credit cards are collected for payment of copayments and such. Time: 46 weeks per year. 32 patient hours per week. While job... Read more (Source: Student Doctor Network)

The code is 0275T however when I select that my EMR auto adds the modifier Q0. I can remove it - but do I? How do you guys bill and get paid for MILD procedure (Source: Student Doctor Network)

Anyone know telepsych companies that allow working abroad?
I understand you can't bill medicare/medicaid when abroad, but are there any telepsych companies that are billing largely insurance or will accommodate a physician who is based internationally? (Source: Student Doctor Network)

I am so f'ing depressed about choosing IM.
I just feel so sad and disappointed at being an IM intern. I have zero passion over the medicine I am practicing. I wish I went into a specialty that was more procedural to be honest, something like anesthesiology or even frankly Radiology. I already have sunk one year into this and will likely waste another year if I decide to reapply and at that point might as well finish residency...oh well...time go write another long ass note that is meant only for billing... Billing and coding
I’m a resident that’s looking for input on billing and coding. I’m in a program that has a small group of doctors, but it’s hard to learn billing and coding for surgery and clinic because it seems so variable from provider to provider. It’s hard to determine what is legit and what is fraud. What are some good resources? Examples of billing that seems confusing: 1. Nail care - some doctors trim any nail that walks through the door and others seem to evaluate each patient for underlying... Read more (Source: Student Doctor Network)

Is private pay adult NP practice feasible ?
Just curious if anyone is doing this & would be willing to share their model/advice/marketing strategies. I’m considering opening a small NP practice for adults. I know that accepting insurance would open accessibility to many and would fill the practice (based on convos with potential referring providers), but I’ve heard such horror stories about working with insurance unless you have the means to hire a billing specialist (i dont) and I’m wondering if there’s a way to do without - even if... Sent a threatening letter from a company for too many 99214
Recently got a letter from a third party company (don't recall the name), comparing my billing codes with others of the same specialty. While it was worded as nicely as possible it indicated that i was billing more 99214 and less 99213 than the average joe psychiatrist. It also suggested I could be audited if this trend continues, it did not specify any insurance companies. I work in a rural hospital and see the sickest of the sick kids, comorbidity is the rule. I'm not concerned my... Read more (Source: Student Doctor Network)

Billing for casting supplies (Q403x)
Do any of you bill for casting supplies when applying a cast? Do insurers usually pay for it separately? The only solid info I've found is that Medicare will pay for it only if the cast is applied in an office setting (Source: Student Doctor Network)

Derms do Rad Onc
Here are the recent office notes from a dermatologist. He's doing all the rad onc billing, as a derm (and getting paid too I presume). Billing for weekly 77427. Billing a daily sim (77280) and G6001. Literally doing IGRT for simple skin cancer. Only in America. This is a Medicare patient. When a Medicare patient gets billed for radiation therapy treatment, does CMS have the bandwidth/subtlety to know that it's derm doing the billing and not rad onc? The majority of radiation fractions... Read more (Source: Student Doctor Network)

Contract renegotiation opinion/input
Working with a specialized psychotherapy practice on a 1099 basis. Currently getting admin assistance in screening and getting paperwork done for new clients, scheduling new appointments, and generation of some kinds of paperwork (ROIs etc). Got assistance with credentialling and they handle billing the one insurance I take. Doing prior auths myself. No benefits otherwise. Currently on a 70/30 split. I've posted revenue numbers before, but so far working three days a week generating revenue... Read more (Source: Student Doctor Network)
