Epidural Reimbursement question
So I got a question for everyone. I'm starting practice fresh outta fellowship, so little new to the area of billing and collections. Say I think someone is a candidate for bilateral L4 and L5 transforaminal ESI. Does that get paid if you bill that? Or is there some limit to how many epidurals you do before insurers stop paying it? What about Medicare/Medicaid specifically as that's what I'll likely be seeing most. We didn't do 4 regions in fellowship hardly ever, though not sure if it... Fraudulent billing? Trauma codes for elective procedures?
How do DPMs get away with billing trauma codes for elective procedures? I've seen DPMs bill for a ORIF talo-tarsal dislocation on a pediatric flatfoot using an arthroereisis implant. Those same people then bill for a subtalar fusion using an arthroereisis implant in adults. I've seen DPMs bill for both a TMT arthrodesis and ORIF of 1st TMT dislocation for a lapidus bunionectomy. Now I hear DPMs are billing subchondroplasty of the talus as an ORIF of talar fracture. When does it end? Who... Fraudulent billing? Trauma codes for elective procedures? How does billing when moonlighting work?
So this may have been discussed in a previous thread, but I wasn't really able to find anyone answering my specific question. From what I've seen on here in a few different sub-forums and threads, residents aren't able to bill for seeing patients until after they they've finished their residency. At the same time, from what I understand, people can start moonlighting as soon as they pass Step 3. So how does this work knowing that when they moonlight they sometimes are working on their own?... Question on Part D billing pain meds
There is a patient getting rxs for 1 LA and 1 SA opiate monthly. We are not sure if his doctor has done a PA. The LA always goes through but the SA sometimes rejects, sometimes goes through. Recently both went through, but the SA was reversed for a mfg change, then rejected when reprocessed. Called the insurance, they say reject is for exceeding morphine equivalent and needs PA. How do you bill for yourself?
Question: Are there any solo doctors here? Do you do your own billing? if so, how does anesthesia billing work? I know that its based on units and each insurance company will pay differently based on units. I see these AMC's offering 520 K salary - infact, I have one contract in hand for 520K, 10 weeks off, all benefits paid, q8 call in a place where I would like to live, but my wife would not. Critical Care Time Billing Question (for the experts)
- I'm unclear as to one particular aspect of critical care time billing: just how long can and should you bill for. Example: Two shifts ago, I took care of a snowbird couple from Canada that both had carbon monoxide poisoning (his COHb% was 19, hers was 15). I had them both sit and suck on 100% NRB facemasks for 3-4 hours while local fire/police went and secured the scene. They recovered in full, and repeat ABGs had them both around 1-2% COHb. Genicular coding
My billing/coding people are telling me that a three nerve genicular block can only be billed as a single nerve block, 64450, one unit. This seems like you should be able to bill 3 units or use -51 modifier, and get paid for 3 nerves Which is it? And, can you use 77002 or is that gone from all nerve blocks now? "Disease management"
So i went to an "endocrinologist" a while back, and they charged me for both the office visit, and they charged me for "disease management" over $200 bucks, with a cose of S0315. The offices which are now all closed across the state claim that they are *two* separate things to have an office visit AND a disease evaluation. I say WTH? I am filling a fraud charge w/my insurance and making a complain w/the state medical board. How can they try to charge for the same thing twice? The billing... PRP My practice has asked me to integrate PRP into my practice. Who of you are doing this? What is your set up. What body parts... Are you using ultrasound and do you need it really with these injections on low body weight athletes? Obviously anatomic localization not too hard on these folks but what is your experience. Billing for one mbb (1/2 a facet joint)?
I did MBBs on a pt for the levels above and below a fusion (L1 and DPR L5). Did not do the MBB immediately adjacent to the pedicle with hardware, so basically I really just denervated 1/2 of two different facet joints... How do you guys bills this? Maximizing billing in c/l psychiatry
So I'm probably going to start a position with a sizeable C/L component. I'm aware of the horrors of trying to make ends meet as a C/L psychiatrist. The unfortunate nature of this position is that clinical incentives are based on RVU collections and not RVUs Billed, which means I'm especially vulnerable to being screwed.
