AMA and EMR and CPT codes
Just got an email from Luminello, saying the AMA strong armed them and other EMRs into needing to pay an annual fee per person who has billing access to their services - assistants, clinicians, etc. ~$20/person. An already low value organization that fails to represent its doctors, and focused on SJW causes for decades and not actual Physicians. Probably spends more money in their aggressive, and sneaky, dues membership mailings. Their wording is frequently Renew now! And they mail to... Read more (Source: Student Doctor Network)

Supervision Questions
Hi all, Long time reader, first time poster. Curious what you all think of this situation. Trying to keep details vague. Recently joined a group with CRNAs. Some days, we are 1:4 billing medical direction (the CRNAs are allowed to procede with starting and ending cases without notifying anyone so I am often running around to be present for induction, which is a whole other discussion). Often, however, we will be at an ASC or late at the main hospital doing our own cases while signing the... Read more (Source: Student Doctor Network)

My Plan to become a Multi-Millionaire Barista/Surf Instructor/Deadbeat
Alright people hear me out (and feel free to criticize as indicated). I'm finishing residency and have an inpatient job lined up at a busy hospital where I will do my own billing (using a biller). In discussions with administration it's my understanding their payors are generally paying in the 140-160 range for the most common inpatient billing code of 99232. New intakes are paying 250-300. For simplicity let's call it 150 and 275. I can dictate my own patient cap and am interested in a cap... Read more (Source: Student Doctor Network)

RFA - procedural/coding question
Let’s say I knew someone who billed 64635 and 64636 and only used two needles to perform that injection (needles placed at L4MB and L5DR). Is there a textbook or article someone could direct me to that outlines how that procedure should actually require 3 needles? The cms guidelines for RFA speak on billing levels, but not needles required to treat said levels. I looked in my rothman text and found nothing, and don’t see anything on google. Disposable nasopharyngoscopes
Our clinic is looking into disposable options for nasopharyngoscopes for head/neck patients. Is there a brand that you like? I heard that the scopes can be charged to the patient's insurance company. What codes do you use when billing for the scopes? Thanks! (Source: Student Doctor Network)

Compensation Before Billing Kicks In
How do other groups pay associates in the firs one or two months of joining before billing actually kicks in? I feel like I'm billing and bringing into the practice more than I am getting at this point. (Source: Student Doctor Network)

In-House EMG / NCS Testing
I'm a physical therapist board certified in clinical electrophysiology (more @ the bottom to answer FAQs). I do EMG/NCS full-time and I have provided over 4,000 studies to MDs/DOs at their practice locations. This has been a mix of “incident to” billing and being directly credentialed with payers (billing under my NPI). 1. Are podiatric physicians able to bill my services “incident to” ? 2. What are the most common reasons DPMs order EMG studies? e.g. insurance requires proof of... Read more (Source: Student Doctor Network)

Inpatient Job Doing My Own Billing
I'm entertaining an inpatient job where I would be reimbursed through my own billings of patients I see. I've been told my patient load can be 14-20, "whatever you pick up". Does this imply I could see more patients if I wanted - not sure how that would even work though. Generally speaking, I know outpatient billing codes have a wide variety of reimbursements based on locale, payor, etc. When entertaining an inpatient job where I do my own billings, how do I even get a vague sense of how... Snf billing help
How often are you guys billing 99309 on your snf patients? I rarely do, I’m almost always billing 99308 and I feel like I’m under billing. Also, I was recently told custodial patients at a nursing home are 99335-6’s with place of service 33. If this is the case I’ve been billing these wrong for longer then I want to think about. How often do you bill a 335 vs 336? I was only using those code for assisted living patients. (Source: Student Doctor Network)

99292 CMS change
Just found out about the new CMS rules requiring the full 30 minutes before 99292 charges. This effectively makes 99291 30-103 minutes now. Nothing says thank you like hitting charge capture for cognitive time spent on a difficult case. Outpatient medicine can now outpace CCM billing with this new change if you have a difficult patient. Just a psa for us based providers about the love CMS is showing us after the pandemic. (Source: Student Doctor Network)

ASC contracting help
There's a group of us who are part time hospital employees who staff a local ASC through a large private practice anesthesia group (no non-complete with hospital). This anesthesia group is now pulling out of the area and the ASC will not have anesthesia staff in about month. The ASC reached out to our core group (4 anesthesiologists) looking for a solution. They are offering to hire us as associate providers (no benefits, vacation) at $200/hr and also let us keep a portion of our billing... Read more (Source: Student Doctor Network)

Help needed with inpatient billing codes
Hi - so I understand billing codes are 99231, 99232, and 99233. Have read about when to bill each, but still feel things are somewhat vague. Can anyone provide examples and/or have a helpful chart that might make things easier for me? Thanks. (Source: Student Doctor Network)

Retina Salary Surveys; Billing to Bonus Ratio
I have something from 2014, but I'm wondering if anybody has anything more recent for vitreoretinal salaries--ideally from ASRS or AAO. I know that many practice owners easily clear $1 million, and that starting salaries are usually in the $350-$400k range--but am looking for something verified/more official. Also, if anybody knows anything about a typical billing to bonus ratio, that would be great. (Source: Student Doctor Network)

Acute rehab billing- help
Being asked to help out my inpatient counterparts to cover a week of inpatient due to need and locums flaking. Haven’t done it in about 11 years. Are the billing codes the same? 99231-3 for followups? What’s the admit/discharge code? Can you bill for team meetings? Anything Specific I need to document in my daily note other than their rehab progress and medical issues? (Source: Student Doctor Network)
